Some gastric dysplasia may have a biopsy result of "atypical glands".

"건진으로 시행한 내시경에서 위전정부의 elevated lesion with central depression이 있고 조직검사에서 atypical gland라고 하여 의뢰된 환자입니다. Outside slide reading에서는 H. pylori-chronic gastritis, active, with intestinal metaplasia (complete type) and atypical regenerating glands로 나왔습니다. 내시경절제술을 시행하였으며 "Adenoma with low grade dysplasia; 1) gross type : depressed, 2) size: 0.5x0.4x0.1 cm, 3) resection margin : free from adenoma"와 같은 최종 병리결과를 얻었습니다.
지금까지 첫 조직검사에서 atypical gland로 진단되었으나 내시경절제술 후 위암으로 최종 진단되었던 예를 몇 번 보여드렸습니다. 이 때문에 "atypical gland는 모두 위암인 모양이다"라고 잘 못 생각하시는 분이 계실지 모르겠습니다. 그러나 오늘의 증례처럼 adenoma로 혹은 neoplastic lesion이 아닌 것으로 나오는 환자도 얼마든지 있습니다. 중요한 것은 첫 내시경의 육안소견일 것입니다. 위암이 의심되는데 atypical gland가 나오면 대부분 위암이지만, 다른 질환이 의심되는 상황에서 atypical gland가 나오는 경우는 위암이 아닐 확률이 높다고 생각합니다.
