위체상부 후벽은 조기위암 진단과 치료가 쉽지 않은 영역입니다. 특히 fundus로 넘어가는 fold에 걸쳐서 위치한 조기위암의 치료는 매우 까다롭습니다. 최근 이 부위의 그야말로 희미한 조기위암을 의뢰받았습니다. 저 같으면 발견하지 못하였을 것 같은 병소인데, 개업가 선생님께서 참으로 잘 발견해 주셨습니다.
의뢰주신 선생님의 내시경 사진이 선명했기 때문에 내시경 재검 없이 즉시 ESD를 하였습니다. 위치때문에 상당히 힘들었습니다. 특히 fundus로 넘어가는 부위의 처리가 쉽지 않았습니다. 내시경 삽입부터 절제 검체 수거까지 60분이나 걸렸습니다 (보통은 30분 전후).

1. Location : high body, posterior wall
2. Gross type : EGC type IIb
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 16 mm (2) vertical diameter, 15 mm
6. Depth of invasion : invades mucosa (muscularis mucosa) (pT1a)
7. Resection margin : free from carcinoma(N) - safety margin : distal 3 mm, proximal 2 mm, anterior 4 mm, posterior 16 mm
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
