작성된 권고문안에 대하여 문헌근거가 어느 정도 있는지, 실제 임상에서 얼마나 유용한지에 대한 전문가의 의견을 수치 혹은 단계화하여 제시해야 한다. 권고문안을 지지하는 문헌을 읽지 않고도 등급만 보고 권고안의 신뢰성이 어느 정도인지 파악할 수 있어야 한다. 문헌근거수준과 권고등급을 따로 제시할 수 있으나, 최근에는 문헌근거수준과 권고등급을 묶은 하나의 system으로 제시하는 방법이 널리 씌이고 있다.
1. 근거수준 (level of evidence; LE): 근거의 수준과 질, 근거의 양 (논문의 수, 대상자의 수, 효과의 크기), 연구 결과의 일관성
2. 권고등급 (grade of recommendation; GR): 권고대로 중재했을 때 바라는 결과가 나올 것으로 확신하는 정도. 효과의 크기(magnitude of effect)에 대한 고려는 없음.
3. 권고수준 (strength of recommendation): 근거수준과 권고등급을 모두 고려하여 하나의 지표로 제시하는 방법. 2011년 위암가이드라인에서 사용한 방법이다.
[부인암 진료권고안(2010)에서 사용한 권고 수준]
근거수준과 권고등급을 모두 통합하여 하나의 지표로 제시하였다. 권고의 등급은 1 (strong)과 2 (weak)로 제시하였고, 문헌근거의 수준에 대해서는 GRADE 방법으로 정한 A, B, C, D에 E (근거가 없거나 분석하기 어려운 경우)를 추가하여 사용하였다.

- 1++: High quality meta-analyses, systematic reviews, or RCTs with a very low risk of bias
- 1+: Well-conducted meta-analyses, systematic reviews, or RCTs with a low risk of bias
- 1: Meta-analyses, systematic reviews, or RCTs with a high risk of bias
- 2++: High quality systematic reviews of case control or cohort studies. High quality case control or cohort studies with a very low risk of confounding or bias and a high probability that the relationship is causal
- 2+: Well-conducted case control or cohort studies with a low risk of confounding or bias and a moderate probability that the relationship is causal
- 2: Case control or cohort studies with a high risk of confounding or bias and a significant risk that the relationship is not causal
- 3: Non-analytic studies, e.g. case reports, case series
- 4: Expert opinion
[SIGN 권고등급]
- A: At least one 1++ directly applicable to target population; or many studies 1+ directly applicable to target population and demonstrating consistency
- B: Many 2++ directly applicable to target population and demonstrating consistency; or extrapolated evidence from studies rated as 1++ or 1+
- C: Many 2+ directly applicable to target population and demonstrating consistency; or extrapolated evidence from studies rated as 2++
