[Mucinous adenocarcinoma]

보만 4형 진행성 위암의 조직형은 대부분 poorly differentiated 혹은 signet ring cell carcinoma입니다. 간혹 mucinous adenocarcinoma도 보만 4형의 모습을 보입니다.

보만 4형 진행성 위암인데 CT에서 유난히 gastric wall thickening이 현저하였습니다. 수술 후 아래와 같은 결과를 얻었습니다. 크기가 무려 20cm 이었습니다. Mucinous adenocarcinoma였기 때문에 mucin lake가 위벽을 두껍게 만든 경우였다고 생각됩니다.

- Size : 20x14.5x2.2 cm
- Depth of invasion : extension to subserosa (pT2b)
- Resection margin: free from carcinoma: safety margin: proximal, 4.3 cm; distal, 0.2 cm
- Lymph node metastasis: metastasis to 5 out of 49 regional lymph nodes (pN1)
- Lymphatic invasion : present
- Venous invasion : present (extramural)
- Perineural invasion : not identified

과거에는 mucinous adenocarcinoma는 예후가 나쁜 것으로 알려져 왔습니다. 최근에는 mucinous adenocarcinoma가 단지 진행된 상태에서 발견되었기 때문에 예후가 나쁜 것으로 봅니다. 즉 stage by stage로 비교하면 non-mucinous adenocarcinoma와 큰 차이가 없다는 말이지요.
