[Abstract] Endoscopic submucosal dissection (ESD) is one of the standard treatments for early gastric cancer (EGC) in Korea, accounting for nearly a quarter of all treated cases. According to a 2023 nationwide survey, the mean patient age was 67.4 years, and the mean tumor size was 1.7 cm. Pathological outcomes showed eCura A in 75.8% and eCura C-2 in 16.7% of cases, with 11.4% requiring additional surgery due to non-curative resection. Mastering ESD is challenging, as it requires a deep understanding of all aspects of gastric cancer diagnosis and treatment, including the interpretation of pathological images and reports. Unlike diagnostic endoscopy or standard endoscopic mucosal resection (EMR), ESD has a steep learning curve. Stepwise technical training for ESD typically consists of five stages: (1) introductory lectures, (2) hands-on training using EndoGEL (or other artificial models), (3) ex-vivo pig stomach ESD, (4) live pig ESD, and (5) patient-based ESD, starting as an assistant. At my institution, self-training is strictly prohibited to prevent potential patient safety hazards, and real-time supervision by ESD education specialists is required.
