1980-90년대의 여러 연구에서 출혈성 십이지장궤양의 재출혈률은 5%/year임이 명백해졌습니다. 그 이후 헬리코박터 제균치료가 재출혈 방지에 탁월한 효과가 있다는 점도 명백해졌습니다. 100%는 아닙니다.
어짜피 확실한 만병통치약은 없습니다. 과거의 치료가 효과가 없었던 것이 아니고 새 치료가 100% 확실한 것도 아니라면 과거와 현재의 지혜를 모두 활용하면 어떻겠습니까? 헬리코박터 시대 이전의 maintenance treatment의 적응증을 말하는 것입니다. 저는 다시 1994년판 Misievicz 편저, Diseases of the Gut and Pancreas (2nd ed) 270쪽을 펼쳐보았습니다.
Suggested criteria for maintenance therapy. Any of these features point to the need for maintenance therapy.
- Frequent relapses (three or more/year)
- Severe and/or more prolonged pain with relapse
- Previous haemorrhage or perforation
- Other serious illnesses, e.g. cardiopulmonary disease
- Age over 60 years; due to risk of a complication
- Smoking
- Large or deep duodenal ulcer at endoscopy, with slow symptomatic response to initial healing dose
- Need for NSAIDs with a known previous ulcer
- Ulcer associated with NSAID medication
헬리코박터 제균치료가 십이지장궤양 환자의 표준치료로 자리잡은 이후에 나온 교과서에서는 십이지장궤양의 maintenance treatment에 대한 언급을 찾아보기 어렵습니다. 그렇다고 과거 maintenance treatment가 필요했던 환자들이 헬리코박터 제균치료만으로 모두 해결되었다고 보는 것도 무리입니다.
저는 헬리코박터 제균치료를 한 후에도 위 적응증에 해당하는 환자, 특히 여러 위험인자를 가진 환자에서 PPI를 이용한 longterm maintenance treatment를 권하고 있습니다. 보통 half dose PPI를 2-3일에 한번 사용합니다. Evidence는 없으므로 expert opinion 정도로 이해해 주시면 감사하겠습니다.
