[AGML ? on screening endoscopy] [AGML ? on screening endoscopy]
AGML (acute gastric mucosal lesion)의 진단은 복합적이다. 위험인자가 있는 환자에서 증상이 있으면서 내시경 소견이 전형적일 때 붙이는 것이다. 위험인자도 없고, 증상도 없고, 내시경 소견도 무척 경미한 환자를 AGML로 붙이는 것은 그다지 좋은 아이디어는 아니라고 생각한다. AGML로 의뢰되었지만 미란성 위염으로 판단하였던 두 증례를 소개한다.
2013년 5월 13일 일본 쿄토에서 일본내시경학회 international session에서 여러나라의 상부위장관출혈에 대하여 논의한 시간이 있었습니다 (link, WORD PDF, 0.2 M). 중국 연자께서는 최근 상부위장관 출혈의 35%가 AGML때문이라고 하였고 특히 고령과 NSAID를 많이 사용하는 환자에서 흔하다고 하였습니다. 이에 대한 장시간의 토론이 있었습니다. 아무래도 중국에서는 multile gastric ulcer를 AGML로 부르는 것 같았습니다. 우리가 사용하는 AGML과는 전혀 개념이 달랐습니다. 중국에서 나오는 자료를 볼 때 주의할 점이라고 생각되었습니다. 2011년 3월 23일 EndoTODAY에서 자세히 언급하였지만 다시 한번 AGML에 대하여 옮깁니다.
Acute gastric mucosal lesion (AGML)은 모호한 병입니다. 전통적으로는 stress ulcer와 비슷하게 취급되었습니다. 1984년 Marrone과 Silen이 쓴 리뷰(PMID6430609)의 제목은 "Pathogenesis, diagnosis and treatment of acute gastric mucosal lesions"입니다. 그러나 첫 문장은 이렇게 되어 있습니다. Stress ulcers are multiple superficial mucosal lesions which occur mainly in the fundus of stomachs of seriously ill patients and should be differentiated from reactivation of a pre-existent ulcer diathesis, Cushing's ulcer following head injury, or drug-induced gastritis. 우리가 막연히 생각하고 있는 AGML과는 분명히 다릅니다.
아래는 어떤 선생님의 강의 슬라이드 내용입니다 (정확히 누구셨는지 기억나지 않음). 아쉬운 점이 없지 않지만 그래도 비교적 잘 정리되었다고 생각됩니다.
- Focal, acutely developing gastric mucosal defects
- Caused by NSAIDs or severe stress
- Usually less than 1cm, circular, small, multiple
- located mainly in the stomach and occasionally in the duodenum
- Range in depth from superficial epithelium (erosion) to deeper lesions (ulceration)
- Are not precursors of chronic peptic ulcers
- Unlike peptic ulcer, found anywhere in the stomach, gastric rugal pattern is essentially normal, and the margins and base of ulcer are not indurated, no thickening and scarring of blood vessels
위 설명에 대하여 저는 몇 가지를 보충하고 싶습니다. (1) 임상적 특징이 들어가야 합니다. 복통과 구토 등 심한 증상이 갑자기 발생합니다. (2) 내시경적 특징도 중요합니다 (Multiple ulcers and erosions, frequently accompanied by hemorrhage). (3) 병소가 주로 1 cm보다 적다는 것은 틀린 말 같습니다. 다양한 크기의 다양한 병변이 중요한 포인트입니다.
AGML은 그 험한 모양에 비하여 거의 완벽하게 호전됩니다. 아래 사진은 매우 심한 AGML이었는데 약간의 ulcer scar를 제외하곤 깨끗하게 회복된 것을 확인할 수 있었습니다.
