합병증의 정의가 명확해야 발생률 자료를 모으거나 대책을 세울 수 있습니다. EMR/ESD 관련 출혈의 정의는 무엇일까요? 많은 논문에서 합병증 발생률이 제시되고 있지만 명확히 정의된 곳은 거의 없습니다.
Jikei 대학의 Tajiri가 2004년 Digestive Endoscopy지에 기고한 내용 중 일부를 옮깁니다 (2004;16(Suppl):S134-S136). 2003년 12월 20일 일본 EMR 컨퍼런스에서 발표된 것이므로 제법 오래된 내용입니다. Postoperative bleeding과 accidental bleeding의 구분이 명확하지 않다는 단점이 있지만 나름 유용합니다.
(1) Intraoperative bleeding: cases requiring special measures such as emergency surgery, intraoperative blood transfusion, or vasopressor therapy and cases where EMR has to be necessarily discontinued because of intraoperative bleeding
(2) Postoperative bleeding : marked bleeding from the ulcer-affected area after EMR, requiring special measures for hemostasis.
(3) Accidental bleeding : if the Hb level falls by 2 g/dL or more in comparison with the last preoperative level, or if any of apparent bleeding or massive melena, etc. is seen.
문헌을 찾아보면 postoperative bleeding만 제시하고 intraoperative bleeding이나 accidental bleeding을 누락시킨 논문이 많은 듯 합니다. "왜 내가 시술한 환자에서는 문헌에 나온 것보다 출혈이 많지?"라고 느끼시는 선생님들이 계실 것입니다. 다 이유가 있습니다.

ESD 후 출혈 위험에 대한 최근 메타분석입니다 (Libanio D. Gastrointest Endosc 2016). "Procedure duration >60 minutes (OR 2.05) and the use of histamine-2 receptor antagonists instead of proton pump inhibitors (OR 2.13) were the procedural factors associated with PPB (post-procedure bleeding), whereas endoscopist experience and pre-procedural proton pump inhibitors were not. Second-look endoscopy was not associated with decreased PPB (OR 1.34; 95% CI, 0.85-2.12)."
그림에서 제시된 여러 인자 중 시술자가 어떻게 해 볼 수 있는 것은 단 한가지였습니다. ESD 후 H2RA 대신 PPI를 사용하는 것이지요. Second look endoscopy는 이번 분석에서도 유용하지 않은 것으로 다시 확인되었습니다.
