Inlet patch는 web이나 stricture를 일으킬 수 있습니다. Plummer Vinson 증후군의 esophageal web도 inlet patch에 의한 것이라는 주장도 있습니다 (Dig Dis Sci 1979). Pain이나 odynophagia도 가능합니다. Laryngopharyngeal reflux symptoms 과 관련되었다는 주장도 있습니다 (Eur Arch Otorhinolaryngol 2010).
이와 같은 inlet patch에 의한 합병증은 위산과 관련된 것 같습니다. 앞서 소개한 리뷰 (Am J Gastroenterol 2004)에는 이런 말이 있습니다. The clinical importance of HGM is limited to symptomatic cases. Symptoms, signs, and complications are acid-related. 흥미롭게도 이와 같은 내용이 담긴 문단의 제목이 'Acid is the Evil Agent'였습니다.
제가 보는 임상적 의미는 이렇습니다. Web이나 stricture는 가능하다고 생각합니다. 그러나 laryngopharyngeal reflux (LPR)는 다를 수 있습니다. LPR symptoms으로 PPI를 사용한 후 증상이 호전되었다면, 이는 위산역류가 줄었기 때문일 수도 있고 inlet patch에서 산분비가 줄었기 때문일 수도 있습니다. 둘을 정확히 구분하는 것은 불가능한 일입니다. 다만 inlet patch의 산분비 감소가 중요하다면 PPI 투약 후 상부식도증상이 즉시 좋아져야 할 것입니다. 사실은 그렇지 않습니다. 그렇다면 이것이 뜻하는 것은 무엇일까요?
제 주장과 달리 2009년 Gastroenterology에는 Argon plasma coagulation of cervical heterotopic gastric mucosa as an alternative treatment for globus sensations라는 흥미로운 논문이 발표된 바 있습니다. 저도 어떻게 의미를 부여해야 할지 잘 모르겠습니다. 추시가 필요한 부분입니다.
