[20110529. Issues on duodenal ulcer (11): more about the risk of overhealing]
오래된 책을 뒤적거리다가 우연히 다음과 같은 문구를 보았습니다. 얼마나 반갑던지... Stenosis may develop for the first time in patients receiving H2RA, and it is conceivable that accelerate healing with these agents may favour the development of stenosis (1994년판 Misievicz 편저, Diseases of the Gut and Pancreas (2nd ed) 292쪽). 참고문헌으로는 Five-year study of cimetidine or surgery for severe duodenal ulcer dyspepsia라는 1982년 논문이 언급되어 있었습니다 (전문보기).
십이지장궤양으로 수술 예정인 환자에게 우선 cimetidine을 투여한 single arm study였습니다. 일부 환자는 수술을 하였고 일부 환자는 약을 잘 유지되었다는 이야기였습니다. 그런데 생각보다 stenosis가 많았습니다. 처음에 없던 stenosis였던 것이지요.
저자들은 이렇게 해석하고 있습니다. In this follow-up study, 5 of 20 patients who underwent surgery did so for pyloric stenosis which developed during cimeticine maintenance therapy. ... The question therefore that has to be answered is whether accelerated healing in response to cimetidine was a factor in such rapidly developing stenosis.
Cimetidine으로 치료하는 상황에서 accelerated healing으로 인한 stenosis 우려가 있다면... PPI에 대해서도 비슷한 고려를 해야 하지 않을까요? (제가 이승복은 아니지만...) 너무 무시되고 있는 영역인지라 저는 큰 목소리로 외치고 싶습니다.
"너무 빠른 치료는 좋지 않다. 최소한 좋지 않을 수 있다."
