[Albendazole과 유사 약제 (mebendazole, flubendazole)]

1. 책에는 mebendazole을 쓰라고 되어 있는데 현장에서는 모두들 albendazole을 쓰고 있을까요?

두 약은 비슷하면서도 다릅니다. 미묘한 차이가 있습니다. Mebendazole과 albendazole은 모두 benzimidazole 유도체입니다. Albendazole은 흡수가 잘 되고 mebendazole은 흡수가 안 됩니다. 우리에게 익숙하지 않지만 flubendazole이라는 약도 있습니다. Mebendazole처럼 흡수가 안 되는 약입니다.

Albendazole

Albendazole은 체내에 흡수되어 간에서 활성형인 albendazole sulphoxide로 바뀝니다. Sulphoxide형은 충체의 포도당대사에 관여하는 tubulin polymerization을 억제합니다.

Albendazole은 보통 식후에 투약합니다. 그러나 고지방식과 함께 약을 복용하면 공복시 복용에 비하여 albendazole sulphoxide의 혈중 농도가 5배 높아집니다. 따라서 조직내 기생충이 target인 경우에는 고지방식과 함께 복용하는 것이 좋습니다. 반면 장내 기생충이 target인 경우에는 흡수를 줄이기 위하여 공복시 약을 복용하는 것이 좋다는 의견도 있는데 이는 명확하지 않습니다. 활성화되지 않은 albendazole이 어느 정도 효과적인지 명확하지 않기 때문입니다.

서양 교과서에는 albendazole이 장내기생충의 치료로서 거의 언급되어 있지 않습니다. 이는 albendazole이 장내기생충약으로 FDA 승인을 받지 못하였기 때문입니다. FDA에서 나온 albendazole 설명서를 보면 neurocysticercosis와 hydatid disease에서만 쓰는 것으로 되어 있습니다.

우리나라에는 albendazole을 만드는 회사가 매우 많습니다. 반면 mebendazole을 만드는 회사는 드물고 게다가 모두 수출용입니다. 근처 약국에는 albendazole만 있고 mebendazole은 없습니다. Flubendazole은 가끔 있습니다.

요컨데 우리는 mebendazole이 없어서 albendazole을 쓰는 것입니다. 반면 미국에서는 albendazole이 소화기 기생충약제로 허가를 받지 못하였기 때문에 mebendazole을 쓰고 있습니다. 미국 교과서 저자들은 mebendazole을 권할 수 밖에 없었을 것입니다. Albendazole과 mebendazole을 모두 쓸 수 있다면 어떤 것을 골라야 할까요? 아마 둘 다 별 문제 없을 것 같습니다. 저는 익숙한 albendazole을 쓰겠습니다.

2. Flubendazole

[2013-9-16. 애독자 질문] 안녕하십니까? 얼마전에 기생충이 있다고(회충같다고 인터넷에서 찾아보고 왔습니다) 찾아온 중국인이 있어 알벤다졸 400mg을 약국에서 사먹으라고 적어 주었습니다. 이후 재미난 일이 있었는데 젤콤이라는 약을 들고 왔습니다. 저도 제일 익숙한 이름의 회충약이라 당연히 알벤다졸 이라 생각했는데 플루벤다졸 이더군요. ~벤다졸이라는 이름이 유사하지만 혹시몰라서 약국에 다시 연락해 알벤다졸을 처방 받게 하였는데요. 이 약도 알벤다졸과 같은 효능이 있다고 할 수 있을까요? 적응도 같은가요?

[2013-9-16. 주인장 답변] 좋은 질문 감사합니다. 두 가지 의견을 드립니다.

1) 회충같은 것과 회충은 다르다고 생각합니다. 회충은 충체가 밖으로 나오는 경우가 매우 드물기 때문에 충체가 항문으로 나왔으면 혹시 tapeworm은 아닌가 의심해야 합니다. 환자 말을 전적으로 믿기는 어렵다고 봅니다. Tapeworm은 납작한 것이 특징입니다. 만약 tapeworm이라면 praziquantel을 써야 하기 때문에 확인이 필요합니다.

2) Flubendazole은 회충에 잘 듣습니다. 상세한 내용은 저도 잘 몰라서 채종일편 임상기생충학(587쪽)의 해당 부분을 옮깁니다.

Flubendazole: Mebendazole의 불소 치환 상사체로서 분말 결정체이다. 장관으로 거의 흡수되지 않으며, 일부 흡수된 것은 104시간에 혈장농도 최고치에 달한 후 10-16시간에 모두 배설된다. 작용기전은 충체의 microtubue 기능과 단백질 대사를 방해함으로써 살충작용을 나타내는 것이다. 투여 용량에서는 거의 부작용이나 독성이 없는 것으로 알려져 있다. 드물게 가벼운 두통, 어지러움, 복통이 나타날 수 있다. 회충증, 구충증, 요충증에 효과적이며, 편충증, 포충증(hydatid disease)과 개회충증에도 사용 가능성이 제시된 바 있다. 어른과 어린이 모두 500mg을 1회 복용한다.
