2009년 ACG white paper를 소개합니다 (Am J Gastroenterol 2009). 우리나라의 관행과 달리 ACG에서는 항혈소판제를 여간해선 끊지 말라고 권하고 있습니다. 어제 소개드린 2009년 ACCF/AHA Practice Guideline와 거의 같은 맥락입니다.
1. Avoid cessation of all antiplatelet therapies after PCI with stent placement when possible.
2. Avoid cessation of clopidogrel (even when aspirin is continued) within the first 30 days after PCI and either DES or BMS placement when possible.
3. Defer elective endoscopic procedures, possibly up to 12 months, if clinically acceptable from the time of PCI and DES placement.
4. Perform endoscopic procedures, particularly those associated with bleeding risk, 5-7 days after thienopyridine drug cessation. Aspirin should be continued when possible.
5. Resume thienopyridine and aspirin drug therapy after the procedure once hemostasis is achieved. A loading dose of the former should be considered among patients at risk for thrombosis.
6. Continue platelet-directed therapy in patients undergoing elective endoscopy procedures associated with a low risk for bleeding.
필자는 2009년 ACG white paper의 내용을 아래와 같이 이해합니다.
1. Coronary stent 환자에서 항혈소판제를 전부 끊어버리는 모험을 하지 말아라.
2. 30일 이내는 매우 위험하니 clopidogrel을 끊지 말아라.
3. Drug eluting stent 환자에서 elective endoscopy는 가능한 1년 이후로 미뤄라.
4. Clopidogrel을 끊더라도 아스피린은 유지해라.
5. 시술 후 가급적 일찍 항혈소판제를 다시 써라. 고위험군에서는 loading dose를 써라.
6. 저위험 시술에서는 항혈소판제를 그냥 유지해라.
아래는 필자의 개인적인 지침입니다.
1. ESD는 가급적 DES 1년 후, BMS 1달 후 시행한다. 급하면 언제든지 할 수 있지만 항혈소판제는 그대로 쓴다.
2. DES 1년 후, BMS 1달 후 시행한다면 clopidogrel은 4일 전부터 끊고 아스피린은 유지한다.
3. 시술 다음 날 통상 용량의 clopidogrel을 다시 시작한다.
즉 조금 늦게 끊는 대신 loading dose를 사용하지 않는 것이 제 방식입니다.
다음은 어떤 cardiologist에게 들은 내용입니다. "요즘에는 drug-eluting stent도 bare metal stent 에 비해서 위험하지는 않다고 생각하는 그룹이 있다. 일본 데이타를 보면 DES vs BMS에서 비심장 수술의 빈도도 비슷하고 생존율도 비슷하였다. 따라서 이 이슈는 계속 정답이 변할 것이다." 여러분, 어떻게 생각하십니까. 저는 또 recommendation이 바뀔 수 있겠다고 생각하고 있습니다. 계속 공부하는 수 밖에...
