[애독자의 추가질문] 다시 한번 이론적으로 삽입법을 정리하고 가다듬는데 큰 도움이 되었습니다. 추가 질문도 드리겠습니다. 혹시 후두삽입에 있어서 피검사자의 해부학적 차이나 혹은 이상으로 인해 통상적인 방법으로 원할치 않은 적이 있으셨는지요? 그렇다면 그런 해부학적 차이를 확인하는 tip이나 주의해야할 특이 후두모양이 있는지도 궁금합니다.
[이준행 답변] 인후부 질병(예: 후두암)으로 치료받은 분의 경우 해부학적 변화가 예상되므로 사전에 병력을 잘 확인할 필요가 있습니다. 그 이외에 해부학적 차이에 따른 tip은 생각나는 것은 없습니다만... 삽입시 방해가 될 수 있는 몇 가지 경우를 소개합니다.

[Gastrograffin을 조충의 치료에 사용]
서민 교수님과 최근 새로 만든 EndoTODAY 기생충학 link에 대하여 메일을 주고 받던 중 일본에서는 diatrizoic acid를 써서 치료하는 방법이 있다는 것을 알게 되었습니다. 장운동을 활발히해서 충체를 완전히 제거하는 방법이라고 하셨습니다. Diatrizoic acid라면 barium을 쓰기 어려울 때 사용하는 radiocontrast agent (Gastrograffin)인데요...... 갑자기 궁금증이 발동하여 짧은 자료 검색을 했습니다.
1) Wikipedia의 D. latum 치료부분입니다. Duodenal tube를 통하여 여러 solution을 주입하여 D. latum 뿐만 아니라 여러 조충을 배출시키는 방법이 소개되고 있습니다. 특히 Gastrografin (diatrizoic acid)이 많이 사용되는 모양입니다.
The standard treatment for diphyllobothriasis, as well as many other tapeworm infections is a single dose of Praziquantel, 5?10 mg/kg PO once for both adults and children. An alternative treatment is Niclosamide, 2 g PO once for adults or 50 mg/kg PO once. One should note that Praziquantel is not FDA approved for this indication and Niclosamide is not available for human use in the United States. Another interesting potential diagnostic tool and treatment is the contrast medium, Gastrografin (diatrizoic acid), introduced into the duodenum, which allows both visualization of the parasite, and has also been shown to cause detachment and passing of the whole worm.

Diphyllobothriasis was previously treated with praziquantel or niclosamide. Praziquantel has some side effects that are very similar to diphyllobothriasis symptoms. Niclosamide is not absorbed in the gastrointestinal tract and has usually no side effects. However, these drugs are not reliable in cutting the scolex loose. If the head is left in the intestinal wall it will regrow. The best method is a diatrizoic acid injection into the duodenal wall. The diatrizoic acid causes all worms to detach leaving nothing behind. Niclosamide or praziquantel should only be used, if endoscopic treatment is not available.

The direct effect of Gastrografin on the cestode may also play a role in expulsion. It is presumed that stimulation of the scolex by a hypertonic solution containing iodine may cause release of the cestode body from the intestinal wall, and that acceleration of peristalsis and the increase of intestinal contents may cause expulsion of the cestode.

예를 들면 stomach high grade adenoma(D13.1)을 in situ로 간주하여 D00.2로 바꾸라는 명령입니다. 진단 코드에 엄청난 변화를 가져오는 일이고, 이로 인한 여파가 충분히 검토되었는지 궁금합니다. 많은 보험회사에서 코드에 기반한 보험급여정책을 가지고 있는데 그로 인한 환자와 의사와의 무의미한 갈등을 어떻게 막을 것인지 대책이 있다는 말을 듣지 못햇습니다. 저는 반대합니다. 옳고 그르고를 떠나서 이러한 문제를 설명도 없이 갑자기 바꾸라는 지침을 내리는 폭력적 행정행위에 반대합니다. 환자에게 어떠한 코드를 부여할 것인지를 '중암암등록본부'에서 지침을 내릴 권한이 있다고 생각되지 않습니다. 그래서 더욱 반대합니다. 코드 시스템의 핵심은 안정성인데 이렇게 지침을 내려가지고는 안정적으로 유지할 수 없다고 봅니다.
