2017년 11월 4일 오전에 있었던 Pariet symposium에 참석하였습니다.

1. Strategy of H. pylori eradication in the era of increasing antibiotic resistance (Jyh-Chin Yang)

연자는 Maastricht V/Florence consesnsus report 중 아래 몇 개의 statement를 언급하였습니다.

In clinical practive when endoscop id performed, RUT is recommended as a first-line diagnostic ttest. RUT is not recommeded as a test for H. pylori eradication assessment.
CLA susceptibility testing is recommended when CLA-based treatment is onsidered as the first-line therapy, except CLA-R < 15%.
After a first failure, culture and standard antimicrobial susceptibility testing (AST) are recommended except if a BQT is considered.
High dose PPI twice daily increase the efficacy of TT. Esomeprazole and rabeprazole may be preferred in areas where the prevalence of PPI CYP2C19 EMs is high.

항생제 내시경 대해서는 자신의 자료를 보여주었습니다 (Yang. CGH 2015).

연자는 standard culture and antimicrobial test에 대한 강한 옹호자였으며 clarithromycin 저항성 판단을 위한 genotyping의 약점을 아래와 같이 요약하였습니다.

Genotypic testing for CLA-R detection cannot well predict the MIC level.
... cannot distinguish the potency of different antibiotics in the same family (e.g. clarithromycin vs erithromycin)
... may cause about 30% false negative results, and may render about 20% of CLA-R patients be treated inappropriately.

하루 4번 투여하는 HDDT (high dose dual therapy)가 유용할 것이라고 합니다. 하루 4번 투여하면 하루 2번 투여하는 것에 비하여 혈중 농도가 낮아지는 기간이 짧아집니다.

제가 가장 인상깊게 들은 것은 자신의 외래에서는 대부분의 환자에서 헬리코박터 항생제 내시경 검사를 한다는 것이었습니다. 제균치료 약제 선정에 있어서 내시경 검사 결과를 고려하여 개인별로 가장 적합한 항생제를 선택하고 있으며, 그 결과 95% 이상의 primary eradication rate를 얻고 있다고 자랑이 대단했습니다. 항생제 내시경 검사 결과가 있으면 1주 치료로 충분하다고 합니다. 이를 "talored approach"라고 부렀습니다. 부러웠습니다.
