[SMC 집담회]

1. Hemophagocytic lymphohistiocytosis (HLH) due to intravascular large B cell lymphoma (삼성서울병원 증례)

허리 통증, 기력저하로 한약을 드시던 70대 여성입니다. 체중 증가, edema, pleural effusion, INR prolongation, thrombocytopenia, LFT 악화 등으로 의뢰되었습니다. 의뢰 후 첫 검사는 아래와 같았습니다.

의뢰 전 MRCP

즉시 transjugular liver biopsy와 bone marrow 검사를 시행하였습니다. 결과는 아래와 같았고, soluble IL-2 receptor 검사도 14,621 U/ml로 상승되어 있었습니다.

이상의 소견을 바탕으로 Hemophagocytic lymphohistiocytosis due to intravascular large B cell lymphoma로 진단하였습니다.

발표를 준비하신 김지유 선생님께서 이 어렵고 무서운 질환을 compact하게 정리하여 주셨습니다.

[백승운 교수님 comment] 한약 복용력이 있어서 초기에 toxic hepatitis를 의심하였던 것 같습니다. 그러나 pancytopenia와 hepatomegaly가 함께 있다는 점은 toxic hepatitis에 맞는 소견은 아닙니다.

2. Enteropathy-associated T cell lymphoma (삼성창원병원 증례)

Celiac sprue의 임상상은 아니었지만 수술 병리 소견에서 villi 소실 등 enteropthy 소견이 있어서 enteropathy-associatd T cell lymphoma로 진단되었습니다. 저는 임상적으로 celiac sprue가 없는 상황에서 enteropathy-associatd T cell lymphoma 진단이 가능한지 질문하였고 박동일 선생님께서는 celicac spure가 없는 subtype도 존재한다고 답변해 주셨습니다.

Recent studies indicate that EATL consists of 2 diseases that are morphologically and genetically distinct and differ with respect to their frequency of association with celiac sprue. The first type of EATL (type 1) is characterized genetically by chromosome 9q31.3 gain or 16q12.1 deletion and is strongly associated with celiac sprue and the HLA-DQ2 haplotype. Interestingly, these lymphomas frequently have a large-cell or pleomorphic cytology and may express CD30. In contrast, EATL type 2 is characterized by chromosome 8q24 gain and, less commonly, by 1q and 5q gains. EATL type 2 is less frequently associated with celiac sprue and the HLA-DQ2 haplotype13,16 and is characterized by monomorphic cytology with frequent expression of CD56.
