A slide by Prof. Gotoda. Mixed type histology 중 undifferentiated type인 경우는 pure undifferentiated type보다 림프절 전이율이 오히려 더 높을 수 있으므로 반드시 undifferentiated type에 준하여 치료해야 합니다. 병리과에서는 mixed type의 경우 각각의 비율이 어떤지를 결과지에 반~~~~~~드시 기재해야 합니다.
A slide by Prof. Gotoda. 일본에서는 differentiated type의 과거 expanded indication이 이제는 absolute indication처럼 간주되고 있습니다.
박준철 교수님. ESD 후 병리결과 의거 수술을 시행한 환자에서 local residual tumor이 있을 확률이 10%, 림프절 전이가 있을 가능성이 10%임을 강조하셨습니다.
일본에서 gastric ESD는 매년 50,000건 시행되고 있습니다. 우리나라에서는 위암 ESD가 매년 15,000건 시행되고 있으며, 위선종에 대한 ESD가 매년 20,000건, 합하여 35,000건 시행되고 있습니다. 일본에서 위선종 ESD는 별로 없습니다. 우리나라의 위선종, 특히 고도선종은 일본에서는 위암으로 간주되고 있기 때문입니다. 그렇다면 인구 수 대비 우리나라에서 ESD가 일본보다 많이 시행되고 있는 셈입니다.
NTT Medical Center는 일본에서 colonic ESD를 가장 많이 하는 병원이라고 합니다.
NTT Medical Center로 ESD를 배우러 오는 외국인을 위한 프로그램을 자랑스럽게 소개하더군요.
Hewett 라는 분이 Olympus luncheon에서 cold snare polypectomy를 강의했는데, 우리가 cold forcep으로 제거하는 수준의 tiny한 adenoma도 모두 cold snare polypectomy로 제거하는 것을 보니 다소 놀라웠습니다. 과잉도 이런 과잉이 없다 싶었습니다.
일본 국립암센터 동경병원 Abe 선생이 좋은 강의를 했습니다. (Quality of ESD; It's time to look back). 제가 질문 겸 comment를 했습니다. 우리나라에는 병리과 의사가 너무 부족하기 때문입니다.
These days, artificial intelligence is a popular topic. Young doctors don't apply for pathology, because they worry to lose their job in the near future. As a result, we already don't have enough pathologists, and it will make a big problem in the near future. You mentioned 2 mm section interval and immunohistostaining for lymphatic invasion. Enough time is required for pathologists to examine all the slides carefully. In my opinion, minimum time for pathologic examination can be a new quality indicator of ESD, and it should be mentioned in the domestic and international guideline. What's your opinion on that?

LCI mode에서 'red in purple' 소견을 통해 병소를 발견하고 BLI-magnification으로 characterization 하는 순서로 검사합니다. Yamamoto 선생님은 WLE를 사용하지 않고 처음부터 LCI mode로 검사한다고 합니다.
[LCI]
LCI mode는 color processing을 통하여 color contrast를 증가시키는 방식입니다.
염증은 purple로, 종양은 red로 보입니다 ("red in purple").
[BLI]
BLI mode는 기존의 blue wave에 추가된 협대역 단파장의 blue-violet이 표면에 가까운 작은 혈관에 흡수되어 표면의 contrast가 강조되는 원리입니다. Olympus 사의 NBI와 유사한 색상이 됩니다. BLI는 단독으로도 유용하지만 magnification을 해야 그 장점이 명확해집니다. (참고: BLI의 원리 - PDF)
BLI는 white light endoscopy의 image를 spectrum으로 나누어 재구성하여 보여주던 FICE와는 달리 white light에 협대역단파장(blue-violet)을 추가하여 영상정보 손상이 없이 그대로 보여주는 방식입니니다. Fujifilm사에서도 FICE는 더 이상 홍보하지 않고 BLI로 옮겨간 인상입니다.
Integrated light source를 보면 R, G, B 이외에 제4의 광원(협대역 단파장, blue-violet)이 추가되어 있음을 알 수 있습니다.
협대역 단파장인 blue-violet만 추가된 것이 BLI이고, blue-violet도 더하고 blue wave도 양을 늘린 것이 BLI-bright입니다.
협대역 단파장 blue-violet 아주 표면에 가까운 혈관에 흡수됩니다.
BLI는 magnification을 하였을 때 가장 효과적입니다. (A). White-light mode. Protruding tumor measuring 18 mm shows an irregular surface in a plain image. (B) BLI mode. Magnified view shows an irregular vascular pattern and an avascular area can be seen partially. Surface pattern also shows irregularity. These finding definitely indicates substantially invasive cancer. (Therap Adv Gastroenterol. 2016)
