청중들의 집중도가 매우 좋은 conference였습니다.
이번 행사는 Youtube를 통하여 실시간 streaming이 되었습니다 (KHIDI USA). 몇 초 정도의 time lag가 있었지만 매끄럽게 잘 진행되었습니다. Internet 환경이 좋으면 매우 선명하게 중계가 되었습니다. Internet speed가 낮은 상황에서는 선명도가 떨어지는 점이 아쉬었습니다.

Stomach cancer is the 3rd leading cause of cancer death worldwide.
미국에서 위암 발생률은 15위 (27,510명)인데, 16위인 식도암(17,650명)보다 흔함에도 불구하고 식도암에 비하여 주목받지 못하고 있습니다. 위암은 Asian American에 많고, 식도암은 Barrett 이라는 뚜렷한 precursor lesion이 있기 때문에 보다 많은 관심을 받는 것 같습니다. Intestinal metaplasia가 있을 때 위암 발생 위험률(OR)은 9.3, East Asian은 15.9이며 (Choi AY, Strate LL, et al - in press), intestinal metaplasia가 caucasian에서도 발견됨에도 불구하고...

1930년에는 미국에서도 위암이 가장 흔한 암이었습니다. 2010년에는 1930년에 비하여 90% 이상 감소하였습니다. 100 consecutive EGD 증례를 분석하였을 때 pure screening 목적은 2명이었습니다.
생각보다 많은 암이 발견되었습니다. 100명 중 5명

한국에서 non-compliance 문제는 아직도 심각합니다.
위암 수술이 매우 centralized 되어 있습니다.
"Open surgery is not standard."라고 강조하셨습니다. Less invasive surgery의 시대입니다.

중국 다음으로 많은 한국인이 미국에서 살고 있습니다. 거의 200백만명입니다. 그 중 35만명 이상이 LA에 살고 있습니다.
Korean American의 위암 문제는 Korea-US collaboration으로 풀 수 있다는 주장이 인상적이었습니다.

손태일 (연세대)

매년 600건 이상의 robotic gastrectomy가 진행되고 있습니다. Blood loss가 적고 hospital stay가 짧고 learning curve effect가 적은 장점이 있으며, longterm oncologic outcome에 대한 데이타가 더 필요한 상태입니다.
Reduced port robotic gastrectomy (3 port)가 standard 5 port laparoscopic surgery에 비하여 lymph node harvest가 많았다고 합니다.

김형일 (분당서울대)

일본 가이드라인은 clinical stage I에 대해서만 언급하고 있으나 우리나라 가이드라인에서는 EGC (strong recommendation, strong evidence) 뿐만 아니라 AGC (moderate recommendation, weak evidence)에서도 laparoscopic surgery를 추천하고 있습니다.

KLASS-01 (Laparoscopic gastrectomy for EGC)

[Results] Among the 1416 patients (mean [SD] age, 57.3 [11.1] years; 940 [66.4%] male) included in the study, the 5-year overall survival rates were 94.2% in the laparoscopic group and 93.3% in the open surgery group (log-rank P=.64). Intention-to-treat analysis confirmed the noninferiority of the laparoscopic approach compared with the open approach (difference, 0.9 percentage points; 1-sided 97.5% CI, -1.6 to infinity). The 5-year cancer-specific survival rates were similar between the 2 groups (97.1% in the laparoscopic group and 97.2% in the open surgery group, log-rank P=.91; difference, -0.03 percentage points; 1-sided 97.5% CI, -1.8 to infinity). Per-protocol analysis results were consistent with the intention-to-treat results for overall and cancer-specific survival rates.

분당서울대병원의 위암 수술 사망률은 매우 낮았습니다. Average mortality는 0.07%, Surgery related mortality는 0.03% (3/7655)였습니다.

Samuel Klempner (Massachusetts General Hospital, Harvard Medical School)

약제의 효과를 반영하는 progression free survival은 동서가 비슷한데, 전체적인 치료의 질을 반영하는 overall survival은 아시아가 우수했습니다. 동양에서 서양에 비하여 care를 더 잘 하고 있다는 것을 보여주고 있습니다.

Korean American의 위암 발생률은 Korean in Korea보다는 낮고 Japanese American보다는 높습니다.

너무 tight한 schedule에 지쳐 산책할 엄두가 나지 않았습니다. 우버로 삼청동 순두부집에 가서 순두부 한 그릇 먹고 호텔로 돌아왔습니다.
