[내시경 용어 - fungating과 ulceroinfiltrative]

[2015-8-26] 과거 자료를 보다가 대장내시경 결과에 "On the AV 5 cm to 9cm, a huge circumferential ulcerofungating mass covered with dirty exudate and showing mucosal firability was noted"라고 씌인 것을 보았습니다.

그냥 "lumen을 상당히 좁히고 있는 circumferential mass가 있다"고 쓰면 충분할 것 같습니다. Fungating은 '곰팡이같은'이라는 뜻인데 저는 저런 곰팡이를 본 적이 없습니다. 내시경 소견을 기술하는 용어로 fungating은 안 쓰는 것이 좋겠다고 생각합니다. 잘못 쓰는 예가 너무 많아서 아예 쓰지 말것을 제안합니다.

1. Fungating에 대하여

피부과 의사와 내시경 의사는 비슷한 점이 많습니다. 피부과 의사는 직접 피부를 보고 진단합니다. 내시경 의사는 내시경이라는 기구를 통하여 점막을 보고 진단합니다. 내시경 의사는 병소 기술 방법을 피부과로부터 배웠습니다.

Fungating이라는 용어는 "fungus-like"라는 의미입니다. 유방암이나 기타 질환으로 피부가 매우 흉한 상태가 되어 곰팡이처럼 보이는 경우에 사용되고 있습니다. 아래 사진을 보시기 바랍니다.

A fungating lesion is a skin lesion that fungates, that is, becomes like a fungus in its appearance or growth rate. It is marked by ulcerations (breaks on the skin or surface of an organ) and necrosis (death of living tissue) and usually presents a foul odor. This kind of lesion may occur in many types of cancer, including breast cancer, melanoma, and squamous cell carcinoma, and especially in advanced disease. The characteristic malodorous smell is caused by dimethyl trisulfide. It is usually not literally a fungal infection but rather a neoplastic growth with necrosing portions. (Wikipedia)

그런데 위내시경에서 이런 병소는 거의 없습니다. 제 나름대로 가장 fungating에 적합한 사진을 골라보았습니다. 그런데 이런 병소조차도 fungating이 아닌 다른 용어로 설명할 수 있을 것 같습니다.

오히려 내시경계에서는 nodular surface와 polypoid growth pattern을 보이는 악성 종양을 기술하는 용어로 사용되고 있습니다. 2022년 1월 10일 발간된 'EndoTODAY 위내시경 삽입과 관찰'의 일부를 소개합니다.

2. Ulceroinfiltrative에 대하여

Ulceroinfiltrative는 ulcerative mass가 주변으로 넓게 infiltration하거나, infiltrative mass에 큰 궤양성 병소가 동반된 경우 사용하면 좋습니다. 즉 ulcerative 부위도 현저하고 infiltrative 부위도 현저한 경우에서 사용하십시요. 아래 사진과 같은 예입니다.

보만 2형 진행성위암의 경우 elevated mass가 있고 중앙의 큰 ulceration이 있기 마련입니다. 융기된 부위가 주변으로 infiltration도 없는데 보만 2형 진행성 위암을 ulceroinfiltrative라고 부르지 마십시오. Mass with deep central ulceration으로 부르시기를 권합니다.
