[내시경집담회]
제 104회 내시경집담회가 2014년 6월 10일 오후 6시 Cha Bio Complex 지하 1층 강당에서 열렸습니다. 새로 만들어진 훌륭한 연구소였습니다.
많은 흥미로운 증례가 발표되었는데 그 중 몇 개만 소개합니다.
1. 이소성 조기위암과 복부 follicular lymphoma
제가 발표한 증례입니다. 많은 고민을 하다가 위암은 ESD로 치료하였고 복부 림프절은 surgical excisional biopsy로 tissue confirm 한 증례였습니다.

[2014-6-13. 애독자 (외과의사) comment 1] 이소성위암과 follicular lymphoma 증례를 잘 보았습니다. 외과의사로 궁금한건 slide에서 보니까 "open" excisional biopsy를 했다고 되어있는데 복강경수술이 아니라 개복수술로 excisional biopsy를 했다는 것인가요? 복강경수술로 LN를 excisional biopsy하면 개복수술로 한것과 비교할때 입원기간도 훨씬 줄고 합병증 가능성도 적을 텐데요. 췌장 뒤쪽의 LN면 technically로 좀 어려 수 있지만 그렇다고 꼭 못할 정도는 아닌데요. 췌장 뒤쪽의 LN절제는 개복수술로 해도 어차피 어려운거고 오히려 복강경수술로 하면 더 쉬울 수 있다고 생각하는데요. 교수님의 관심사항은 아닐수 있어서 죄송하지만 참 궁금합니다.
[2014-6-13. 이준행 답변] 좋은 질문 감사합니다. 췌장 뒤쪽 LN에 대하여 어떤 수술적 접근이 좋을지 저야 잘 알지 못하는 상황이었습니다. 외과 선생님께 문의하였더니 개복이 좋겠다고 하여 개복하였을 뿐입니다만... 죄송합니다. 제가 답을 잘 하기 어려운 상황입니다. 외과 선생님들 중에도 laparoscopy를 좋아하는 분도 계시고 개복을 좋아하는 분도 계시는 것 같습니다. 생각해보니 제가 의뢰하였던 선생님은 laparoscopy를 아주 좋아하는 분은 아니셨습니다. 좋은 comment 감사합니다. 잘 보았습니다.
[2014-6-13. 애독자 (외과의사) comment 2] 외과의사가 내과로부터 이런 consult를 받는다면 (예를 들면 이런 환자의 췌장주위 LN 절제) 수술법을 고민할 것 같습니다. 복강경수술과 개복수술은 환자의 경과에 큰 차이가 있다는 것을 아니까 더욱 그럴 것입니다. 그러나 췌장주위의 수술은 technically challenging하니까 외과의사는 고민을 할 것입니다. 외과 선생님께서 여러가지를 종합적으로 고려해서, 특히 환자안전의 입장에서 최선의 결정을 하신 것 같습니다. 때로는 환자에게 너무 잘 해주려고 하는 것보다는 종합적으로 판단해서 냉정하고 보수적으로 결정하는 것이 좋을수 있다고 느낍니다.
질문 하나 더. Fecal transplantation의 적응증은 아직까지 pseudomembranous colitis 뿐인가요?
[2014-6-13. 이준행 답변] 그렇습니다. 아직까지 C. difficile에 의한 refractory pseudomembraneous colitis가 유일한 적응증으로 생각하면 될 것 같습니다. 관련하여 New York Times 기사의 일부를 옮깁니다.
"One of Dr. Khoruts’s concerns about the procedure is that many people assume it can be used for a variety of intestinal problems. He cautioned that, so far, the only real evidence is for C. difficile. Eventually, he said, if researchers can determine which bacteria are crucial, it should become possible to create products containing them, and to spare everyone the unpleasantness of dealing with stool specimens."
2. 난치성 GAVE
이화여자대학교에서는 APC를 이용한 ablation 치료 후에도 출혈이 지속되었던 증례를 발표하였습니다. 참 어려운 증례였다고 생각하였습니다. 사실 저는 뾰족한 수를 생각해 낼 수 없었는데 여러 교수님들이 좋은 말씀을 해 주셔서 많이 배웠습니다. 역시 집담회는 자주 참가하고 볼 일입니다.
GAVE에 대한 APC 치료를 조금 더 강하게 하는 방법에 대한 제안이 있었습니다 (조주영 교수님). Power를 80-100 W까지 올려서 강하게 시행하여 fibrosis가 일어나게 하면 좋다는 comment였습니다. 위 전정부 전부를 한꺼번에 소작하기는 어려우므로, 25%씩 영역을 정하여 순서대로 치료하는 것도 좋은 방법 같습니다 (박재명 교수님). GAVE 환자에서 propranolol을 쓰면 좋다는 comment도 있었습니다 (함기백 교수님). GAVE는 참 치료하기 어려운 병이라는 것을 다시 한번 느낍니다. 앞으로 제게 이런 환자가 오면 저도 propranolol을 시도해 보려고 합니다.
2014년 3월 24일 EndoTODAY에서 소개한 바 있는 GAVE 관련 자료를 아래에 옮깁니다.
Gastric antral vascular ectasia (GAVE) is an unusual but interesting cause of chronic gastrointestinal blood loss and anemia. GAVE was first described in 1953 by Ryder in the gastrectomy specimen of an elderly woman, as 'erosive atrophic gastritis with marked venocapillary ectasi'. In endoscopy, GAVE has unique appearance characterized by prominent erythematous stripes radiating in a spoke-like fashion from the pylorus to the antrum. Jabbari called it as 'watermelon stomach. GAVE is commonly misinterpreted as antral gastritis.
It is most common in the eldery women. It may be associated with cirrhosis in about 30% of cases. Common presentation is iron-deficiency anemia secondary to occult blood loss. Overt gastrointestinal bleeding in the form of intermittent melena and hematemesis is also possible. Histology (not necessary for a diagnosis) usually shows fibromuscular hyperplasia of the lamina propria, intravascular fibrin thrombi and an increase in the mean cross-sectional area of the lumen in mucosal vessels.

3. Fecal transplantation
인하대학교에서는 난치성 pseudomembraneous colitis에 대하여 fecal transplantation을 시행하여 호전된 증례를 발표하였습니다. 환자 따님의 변을 잘 preparation하여 대장내시경을 통하여 환자의 상행결장에 뿌린 증례였습니다. 즉시 증상이 호전되었다고 합니다.
관련하여 흥미로운 토론이 있었습니다. 어떤 donor가 좋은지 질문이 있었는데 microbiota는 시간에 따라 바뀔 수 있으므로 함께 사는 사람의 변을 이용하는 것이 좋다는 발표자의 설명이 있었습니다. Universal donor에 대한 자료는 부족한 듯 합니다. Upper tract로 주는 방법과 lower tract로 주는 방법은 각자의 장단점은 있으나 전체적으로는 큰 차이가 없다는 쪽으로 논의되었습니다.
Fecal transplantation에 대해서는 2013년 NEJM 논문이 유명한 Dutch randomized trial입니다 (아래 그래프 참조). 항생제를 사용한 경우보다 월등히 효과가 좋다고 합니다.
관련하여 New York Times 기사의 일부를 옮깁니다. Dutch randomized study 이전에 여러 연구자들이 많은 경험을 하고 있었기 때문에 예상하였던 결과이고 놀랍지 않다는 것이었습니다. 다들 알고 있던 내용 같습니다. 저만 빼고...
"The study was originally meant to include more patients, but it had to be cut short because the antibiotic groups were faring so poorly compared with the transplant patients that it was considered unethical to continue.
The results come as no surprise to doctors who have tried the procedure. Dr. Alexander Khoruts, a gastroenterologist at the University of Minnesota, said he had performed the transplants in more than 100 patients with C. difficile. He said that it worked the first time in 90 percent, and that the other 10 percent were cured with a second treatment."
심지어는 집에서 스스로 fecal transplantation을 하는 DIY (Do It Yourself) 방법을 소개하는 인터넷 싸이트도 있었습니다. 제목은 The Power of Poop. 우리말로 "똥의 힘"이었습니다. 살짝 걱정되었습니다.
