[ColonTODAY 080 - 조직검사: intramucosal cancer를 어떻게 볼 것인가?]
내시경 조직검사에서 " INTRAMUCOSAL ADENOCARCINOMA, WELL DIFFERENTIATED with villous adenoma with high grade dysplasia component"로 의뢰되었으며, 육안소견 참고하여 수술을 시행하였습니다.
Sigmoid colon, anterior resection:
. Adenocarcinoma, moderately differentiated
1. Location: sigmoid colon
2. Gross type: fungating
3. Size: 2.3x2 cm
4. Depth of invasion: invades muscularis propria(pT2)
5. Resection margin: free from carcinoma, safety margin: proximal, 12 cm ; distal, 3 cm ; circumferential(radial), >10 mm
6. Regional lymph node metastasis : No metastasis in all 10 regional lymph nodes(pN0) (0/10: pericolic, 0/10)
7. Lymphatic invasion: not identified
8. Venous invasion: not identified
9. Perineural invasion: not identified
10. Tumor budding : negative
11. Micropapillary component: no
12. Tumor border: pushing
13. Pathologic staging: Pending
치료야 별 문제없이 적절히 이루어졌지만, 왜 병리결과에 구태여 "intramucosal"이라는 말이 들어갔는지 의문입니다. 위내시경에서는 이런 경우가 별로 없는데 대장내시경에서는 가끔 이와 비슷한 경우를 만났던 것 같습니다. 혹시 이유를 알고 계신 분은 저에게 귀뜸해 주시면 감사하겠습니다.
