조기위암 내시경치료 후 450um 점막하층 침윤이 있어서 환자와 수술 여부에 대하여 진지하게 상담을 하였습니다. 경과관찰을 하기로 한 환자의 뜻에 따라 추적검사를 하였는데 간의 비특이적 소견이 보였습니다. 혹시나 하는 마음에 PET를 처방하였는데 우연히 발견된 대장암 의심 병소가 발견되었습니다. 대변 검사만 해 보았지 대장내시경 검사의 경험은 없는 분이셨습니다.
대장내시경을 시행하였고 조직검사는 tubular adenoma with HGD였으나 수술을 의뢰하였습니다. Single incision laparoscopic anterior resection of the sigmoid colon이 시행되었습니다.
