[결핵성 식도염. Tuberculous esophagitis] - 終

우리나라에서 결핵은 여전히 중요한 질환입니다. 식도결핵이라는 disease entity를 알고 계시면 언젠가 도움되는 경우가 있을 것입니다. 중부식도에 궤양성 병소가 있는데 GERD나 암이 아니면 식도 결핵을 의심해 보아야 합니다.

1) Clinical symptoms of esophageal tuberculosis
- Systemic symptoms: wt. loss, fever, chill
- Local sx. : chest pain, dysphagia(m/c),cough

2) Endoscopic findings of esophageal tuberculosis
- Ulcerative lesion (m/c): mid-esophagus,irregular outline and a grayish base with an irregularly infiltrated edge.
- Infiltrative growth: hyperplastic and granular type lesion
- Tumor-like growth with stricture: can be confused with esophageal cancer

3) Radiologic finding: Nonspecific changes associated with LN or contiguous organ
- Subcarinal LN enlargement: most common finding on chest CT
- Lung lesions, lymphatic lesions and complications such as BEF

매우 특이한 식도 결핵. Tumor-like growth with stricture

[증례보고] Multidrug-Resistant Tuberculous Mediastinal Lymphadenitis, with an Esophagomediastinal Fistula, Mimicking an Esophageal Submucosal Tumor (Kim DW. Clin Endos 2016)라는 긴 제목의 증례보고를 하였습니다.

Odynophagia를 주소로 내원한 환자인데 처음에는 식도 SMT로 추정되었으나 CT에서 mediastinal lymph node가 커져 있어 의뢰된 분입니다. 재검에서 식도 SMT-like lesion의 중앙에서 함몰부(fistula ?)가 보였고 EBUS-TBNA에서 AFB staining은 음성이었으나 PCR 양성이었고 배양에서 다제내성 결핵(INH 내성, RFP 내성)이 확인되었습니다.

2013년 6월 11일 강남성모병원에서 제 100회 내시경학회 집담회가 열렸습니다. 제가 painful swallowing with esophageal SMT-like lesion이라는 제목으로 발표하였습니다 (순서). 현진해, 정인식 교수님께서 우리나라 소화기내시경학회 역사에 대해 추억해 주시는 아주 뜻깊은 시간도 있었습니다.
