[UlcerTODAY 013. Candida-associated SMT-like gastric ulcer]

Candida-associated gastric ulcer looks like SMT with dirty white exudative cap. It can be improved with PPI (without anti-funagal treatment).

Candida 감염에 의하여 ulcer가 생겼는지, ulcer가 있고 Candida가 발생한 것인지 전후관계를 알 수 없습니다. 그러나 모양이 전형적이고, PPI만 쓰고 항진균제 사용하지 않아도 깔끔하게 좋아지는 특징이 있습니다. 첫 조직검사에서 gastric candidiasis가 나왔지만, 전형적 소견이마 PPI만 투약하였고 완전히 좋아졌습니다.

Candida 감염이 동반된 궤양을 설명한 어떤 저널 (Hirasaki S. J Gastroenterol 1999;34:688-693)의 일부를 옮깁니다. "Candida-infected gastric ulcer should be suspected in patients with a gastric submucosal tumor-like lesion with a thick, yellowish-white coated ulcer of unclear border on its summit, and this lesion should be distinguished from malignant diseases." 우리 증례를 그대로 설명해 주는 것 같습니다.

아래는 제가 경험한 또 다른 증례들입니다. 다들 거의 비슷하게 생겼습니다.

'위궤양 + Candida'였고 궤양약만 써도 호전되었습니다. Follicular lymphoma도 가지고 있던 환자입니다.
