[EsoTODAY 056 - Aortoesophageal fistula due to esophageal cancer (s/p esophageal stent and CCRT)] - End of document
65세 남성 (2018년)입니다. 식도암이 massive하였고 PET에서 주변 림프절 전이가 있었습니다. Esophageal stent 후 CCRT를 하고 수술을 계획하였습니다.
중간에 hematemesis가 있었고 내시경에서 massive blood oozing이 관찰되었습니다. CT angiography에서 esophageal stent 근위부에 연하여 aorta로부터 oozing이 보였습니다. 즉시 aortic vascular stent를 시행하였습니다 (TEVAR).
다음 날 Ivor-Lewis 수술을 시행하였습니다.
Esophagus and upper stomach, Ivor Lewis operation:
- Status post neoadjuvant concurrent chemoradiation therapy
- Diffuse mucosal and submucosal necrotizing inflammation with ulceration granulation tissue (size: 14x6cm)
- Transmural chronic inflammation and fibrosis
- No residual tumor
- metastasis to 1 out of 21 regional lymph nodes without extracapsular extension (size of the largest metastasis: 5 mm)
추가 치료 없이 경과관찰 중입니다. Aorto-esophageal fistua를 빨리 발견하고 신속히 vascular stent를 하고 수술까지 진행할 수 있어서 다행이었습니다.
