[일원내시경교실 목요점심집담회 2015-11-19]

1. 매우 큰 GIST

Melena로 시행한 내시경에서 위 체중부에 큰 SMT가 있고 fundus와 high body에 각각 궤양을 동반한 종양이 있었습니다. Multiple SMT with central ulceration이었으므로 multiple GISTs, lymphoma 등을 의심하였습니다. 조직검사에서 c-kit positive GIST로 나왔고 CT에서는 여러개의 GIST가 아니라 매우 큰 하나의 GIST로 나왔습니다. 큰 GIST가 위의 여러 곳을 눌렀던 경우로 판단됩니다.

2. 대장암

상행결장 대장암인데 ESD 후 깊은 것으로 나와 수술을 하였고 no residual tumor가 나왔습니다. 제가 ESD 한 것은 아니지만.... (저는 대장 ESD는 하지 않고 있습니다).

Adenocarcinoma, well differentiated :
1) size of carcinoma: (1) longest diameter, 12 mm (2) vertical diameter, 7 mm
2) invasion into submucosa: present(depth of submucosal invasion: 2200 ㎛)
3) lymphovascular invasion: not identified(N)
4) perineural invasion: not identified(N)
5) tumor budding: not identified(N)
6) resection margins: free from carcinoma(N) (0.02 cm apart from deep resection margin)

3. 대장암

Splenic flexure 근방 대장암인데 ESD 후 경과관찰하였습니다. 제가 ESD 한 것은 아니지만.... (저는 대장 ESD는 하지 않고 있습니다).

치료보다는 진단이 중요한 환자일 것 같습니다. White light endoscopy 사진을 보면 거의 발견하기 어려운 병소입니다. 세상에 고수가 많은 것 같습니다.

ADENOCARCINOMA, WELL DIFFERENTIATED :
1) size of carcinoma: 1.6x1.0 cm
2) invasion into submucosa: present (depth of submucosal invasion: 400 ㎛)
3) lymphovascular invasion: not identified
4) perineural invasion: not identified
5) tumor budding: negative
6) negative resection margins (100㎛ apart from deep resection margin)
