[ColonTODAY 062 - Inverted colonic diverticulum]
Inverted diverticulum은 polyp과 구분하기 어렵습니다. Polyp이라 생각하고 polypectomy를 하면 천공이 발생할 수 있으니 늘 걱정입니다.
Inverted diverticulum에 의한 polyp은 주변 점막의 물결모양 주름이 특징입니다. Polyp의 상단을 조심스럽게 눌러주거나 대장에 공기를 넣으면 polyp이 뒤집혀 diverticulum으로 보이기도 합니다.

사실 이렇게 되면 쉽습니다. 문제는 잘 뒤집히지 않는 경우입니다.
A 42-year-old man with a history of mild, intermittent pain on the lower left side of the abdomen presented with an episode of painless hematochezia. The physical examination was normal, and his hematocrit was 44 percent. Diverticulosis of the sigmoid colon (Panel A) was revealed during flexible sigmoidoscopy for evaluation of the bleeding. There were multiple diverticula and a 1-cm sessile polypoid lesion (Panel B) that was the same color as the surrounding mucosa. The polyp vanished with the application of gentle pressure from the tip of a closed pair of biopsy forceps, confirming that the lesion was merely an inverted colonic diverticulum (Panel C). These pseudopolyps are typically surrounded by diverticula. The diagnosis can usually be confirmed by paying close attention to the color of the lesion and determining whether the lesion disappears with the application of gentle pressure. The patient's gastrointestinal bleeding was attributed to diverticulosis of the sigmoid colon. He has since remained asymptomatic with the use of a high-fiber diet and antispasmodic agents. 교수님 안녕하십니까? 엔도투데이로 공부하는 내과개원의 입니다. 대장내시경에서 inverted diverticulum 과 polyp 감별이 애매해서 문의드립니다.
(1) Mid-ascending colon (70cm from AV). 근처에서 공기를 주입하니, 바로 뚜렷한 게실모양으로 바뀌었습니다.
(2) Distal ascending colon (62cm from AV). 공기 및 물을 주입을 해도, 뚜렷하게 게실모양으로 바뀌지가 않아서, inverted diverticulum 인지, polyp 이 combine 되어 있는 건지, 구분이 애매하였습니다.
두번째와 같은 경우처럼, polyp 과 감별이 애매한 경우에는 어떻게 plan 을 세워야 하는지 문의드립니다. 감사합니다.

어려운 질문입니다.
물결무늬에 집중하고 표면의 성상을 잘 살펴 neoplastic polyp인지 확인하려는 노력이 중요할 것 같습니다. 확대내시경이 아니라도 근접하여 pit pattern이나 microvascular pattern을 살피거나 느끼면 더욱 좋을 것입니다. 공기를 천천히 넣고 빼면서 모양의 변화를 살피고 biopsy forceps로 약간 눌러보면 대부분 구분할 구 있다고 생각합니다.
보이면 자른다는 단순한 전략은 위험합니다. Inverted diverticulum을 절제하면 천공이 발생하기 때문입니다.

[2022-1-18] 최근 SMC 목요내시경집담회 증례 중 inverted diverticulum이 있어 사진을 검토해 보았습니다. 진단에 동의합니다. 물결무늬가 있는 것도 그렇고...
검사하셨던 선생님은 이렇게 말씀을 하셨습니다. "사실 용종이 넘어갈까봐 뒤로 가서 injection을 하고 왔더니 없어져서 당황했던 케이스입니다. 고년차선생님을 불렀더니 inverted diverticulum이라고 하고 가셨고. 조직검사로 떼어냈으면 천공이 될 뻔 하였습니다. 중국 논문에 점막하주입을 해서 딤플링이 생기는 것으로 inverted diverticulum을 진단했다는 내용이 있었습니다.
