[EsoTODAY 014 - Columnar-lined esophagus (CLE)]

전적으로 개인 주장이지만... 1 cm 미만의 ultrashort segment Barrett esophagus의 임상적 의의가 명확하지 않으므로 큰 의미를 부여하지 않고 조직검사도 할 필요가 없다고 설명드린 바 있습니다.

아래 사진과 같이 hiatal hernia가 있는 환자에서 1cm 가량의 short segment Barrett esophagus를 의심하고 조직검사를 하는 경우가 있습니다. 이 환자의 경우 Barrett esophagus 의심 부위에서 조직검사를 했는데 gastric mucosa showing chronic gastritis, active, with lymphoid aggregate로 나오고 intestinal metaplastia는 없었습니다. 따라서 길이 여부를 떠나서 Barrett esophagus의 조직학적 증거는 없습니다.

Short segment columnar-lined esophagus (CLE)로 부를 수 밖에 없고 임상적 의의는 hiatal hernia에 준한다고 생각합니다. GERD 증상이 있으면 투약하고 없으면 그냥 검진 내시경 차원에서 1-2년 후 재검을 하면 됩니다.

70세를 고려하면 glycogen acanthosis는 정상범위입니다. 약간의 sliding hiatal hernia가 있으면서 화면의 8시 - 12시 방향의 squamous epithelium이 columnar metaplasia를 보입니다. 한쪽 방향으로 올라온 short segment Barrett esophagus일 수도 있고 columnar-lined esophagus (CLE)일 수도 있는 소견입니다. Dysplasia를 의심케하는 focal lesion은 없습니다. Biopsy site는 정하기 어렵지만 squamous island를 피하여 squamocolumar junction보다 몇 mm 아래를 택하여 2개 정도 하면 무난합니다. (아래 사진 참조)
조직검사 결과는 "Acanthotic squamous epithelium and gastric mucosa showing chronic gastritis, inactive, without intestinal metaplasia"였고 intestinal metaplasia가 증명되지 않았으므로 Barrett esophagus로 부를 수 없고 일단 columnar-lined esophagus (CLE)로 부르면 적당하겠습니다. 조직검사의 false negative로 인하여 바렛 식도가 CLE로 과소평가된 것일 수 있으나 어짜피 짧은 바렛 식도는 임상적 의의가 거의 없는 것을 생각하면 조직검사 2개면 충분할 것 같습니다. 향후 추적검사에서 변화가 없으면 더 이상의 조직검사는 필요치 않습니다.
