1. Introduction

Gastric varices are found in about one in five patients with portal hypertension. About 5% to 10% of patients with gastric varices may not have esophageal varices.

Gastric varices are more difficult to detect by endoscopy especially if they are small and isolated. Small varices in the fundus are often mistaken for a mucosal fold. Their identity as varices is based on their shape (grapelike clusters) and their bluish tinge. [Ginsberg textbook 2005]

Sometimes, gastric varix can be mistaken as a SMT. Never try to take biopsy from suspicious gastric varix. It may be fatal.

식도 정맥류는 뚜렷한데 위 정맥류는 다소 애매합니다. Fold의 불규칙한 thickening이 약간의 varix일 수 있습니다.

2. 위정맥류 분류법

위정맥류 출혈의 일차예방법에 대한 자료는 충분하지 않습니다.

M/38 (2015). 위정맥류가 현저하였고 식도 정맥류와 portal hypertensive gastropathy는 상대적으로 경미하였습니다. 그런데 몇 년 후 hematemesis가 있었고 출혈부위는 식도였습니다. Red color sign이 현저하면서 그곳에서 출혈이 있어서 EVL을 했습니다. 위정맥류와 식도정맥류가 함께 있는 환자에서 어느 부위로부터 출혈할지 예측은 어렵습니다.

3. 위정맥류 출혈의 내시경 치료

위 정맥류의 대표적인 내시경치료법은 histoacryl injection입니다. Band ligation은 정맥벽이 찢어지는 수가 있어 매우 주의해야 합니다. Ethanolamine은 거의 사용하지 않습니다 (인도에서는 아직도 조금 사용한다고 함). Histoacryl injection이 실패하면 TIPS를 고려할 수 있고 BRTO가 secondary option입니다.

Histoacryl (N-butyl-2-cyanoacrylate; 합성 glue. 물과 피를 만나면 금방 굳는데, lipiodol과 섞으면 조금 늦게 굳습니다)이 출혈 조절에 유용합니다. Red spot이 보이면 조금 떨어진 곳에 주사를 합니다. 시술자의 눈을 보호해야 합니다. 눈에 들어가면 실명할 수 있습니다. 안경을 낀 의사도 보안경을 써야 합니다.
위정맥류 Histoacryl 치료 증례 Gastric varix로 인한 출혈이었고 Histoacryl injection 으로 지혈되었음. Histoacryl에 lipiodol을 섞어 사용하므로 X-ray에서 주사부위를 발견할 수 있습니다.

4. BRTO

Balloon-occluded retrograde transvenous obliteration (BRTO) is one of treatment methods for gastric varix. Ethanolamine oleate is commonly used as the sclerosing agent.

5. Secondary prophylaxis after initial hemostasis

Repeated EVO reduces GV re-bleeding. 현재 출혈이 없는 상태에서 괜히 Histoacryl injection을 시도하다가 출혈될 수 있으므로 주의해야 합니다.

Non-selective beta blocker: 하나의 작은 임상연구에서는 repeated EVO + NSBB가 negative 결과였지만, 실제 임상에서는 식도정맥류가 종종 동반되어 있다는 점을 고려하여 beta-blocker를 사용하고 있습니다.

1) 출혈 환자의 내시경 도중 gastric varix 출혈이 발견되면 담당 교수에게 즉시 연락해 주시기 바랍니다. Histoacryl injection은 상당한 숙련이 요구되는 술기이므로 담당 교수의 감독 없이 혼자 시행하지는 마십시오.

2) 삼성서울병원에서는 BRTO를 다른 어느 병원보다 활발히 이용하고 있습니다.
