[위염의 정의] - 終

위염(gastritis)은 병리 용어입니다. 위점막에 염증 세포가 침윤된 상태를 말합니다.

급성 위염은 고유판이나 상피세포층 안에 소수의 중성구가 흩어져 보이고 미란이 있는 경우에는 중성구가 더 많이 침윤하고 위 내강으로 섬유소를 동반한 화농삼출물이 배출됩니다.

임상적으로 급성위염을 정의하는 것은 어렵습니다. Harrison 교과서 20판 2241page 표에서 acute gastritis를 아래와 같이 나누고 있습니다. 모두 infectious하다고 보는 것이지요

만성 위염은 주로 림프구 침윤을 보입니다. 소수의 림프구는 정상 위점막에 존재합니다. 따라서 어느 정도부터 비정상인지 모호합니다. 임상에서는 상복부 불쾌감을 설명하는 병명으로 남용되고 있습니다. 내시경의 발달과 Helicobacter pylori로 인해 위염의 개념도 많이 달라졌습니다.

위염과 구별되는 용어로 위병증(gastropathy)이 있는데, 염증세포 침윤이 미약한 위점막 손상을 뜻한다. NSAID에 의한 점막손상은 위병증에 해당합니다. 그러나 이 둘의 구분은 명확하지 않습니다.

정상 위점막과 위주름은 다양합니다.

위주름은 위체상부부터 위체하부까지 linear한 경우가 많이만 이 환자처럼 그물 모양인 경우도 있습니다. 물론 정상입니다.
