APASL consensus definition: The ACLF is an acute hepatic insult manifesting as jaundice (serum bilirubin :5 mg/dl) and coagulopathy (INR >1.5) complicated within 4 weeks by clinical ascites and/or encephalopathy in a patient with previously diagnosed or undiagnosed chronic liver disease/cirrhosis, and is associated with a high 28-day mortality.
ACLF is a syndrome in patients with chronic liver disease with or without previously diagnosed cirrhosis which is characterized by acute hepatic decompensation resulting in liver failure (jaundice and prolongation of the INR) and one or more extrahepatic organ failures that is associated with increased mortality within a period of 28 days and up to 3 months from onset
Definition은 중요합니다. Communication에 문제를 일으킬 수 있기 때문입니다.
삼성서울병원 경험에 의하면 ACLF type에 따른 차이가 있었습니다. A는 초기의 고비를 넘기면 문제가 없었는데, B나 C는 90일 이후에도 mortality가 발생하였습니다.
