[Dr. Sinn's LiverTODAY 020 - 유럽간학회 가이드라인]
유럽간학회에서 두 개의 새로운 가이드라인을 발표했습니다.
추후 꼼꼼히 읽어보실 것을 권유드리며, recommendation을 살펴보고 나름 가장 중요한 것이라고 생각하는 것을 선정해 보았습니다.
1. ALF
1) Development of encephalopathy is of key prognostic importance, with onset indicating critically impaired liver function. In subacute presentations, even low grade encephalopathy may indicate extremely poor prognosis (evidence level II-2, grade of recommendation 1).
→ 해설: 급성간부전에서 encephalopathy의 grade가 진행하는 것은 아주 아주 안좋은 신호입니다. 응급 간이식을 머뭇거리다가 응급 간이식의 기회가 지나가는 경우도 흔합니다.
2) Plasma exchange in RCT, has been shown to improve transplant-free survival in patients with ALF, and to modulate immune dysfunction (evidence level I, grade of recommendation 1).
→ 해설: 급성 간부전에서 혈장교환술의 권고등급이 상향되었습니다. 지난번 제가 posting할 (LiverTODAY 004) 때만 해도 심평원에서 삭감 통지를 받았다고 말씀드렸었는데, 이의 신청 후 이의가 받아들여졌다고 연락을 받았습니다. 삼성서울병원에서는 이후 약 12예 정도 증례경험이 있으며, 자연회복된 분도 있고, 이식받은 분도 있고, 사망한 분도 있습니다. 아직 짧은 경험이지만 감염 합병증을 겪는 분들이 많아 주의를 요하고, 혈장 교환술 중 hypocalemia 등 e' 도 매우 면밀히 살펴야 하는 것 같습니다.
3) Early institution of extracorporeal support (RRT) should be considered for persistent hyperammonaemia, control of hyponatraemia and other metabolic abnormalities, fluid balance and potentially temperature control (evidence level III, grade of recommendation 1).
→ 해설: 전통적인 RRT의 적응증보다 빨리 시작하는 것을 권고하고 있습니다. 근거수준은 부족하지만 recommedation 권고등급은 높습니다. 저도 RRT를 ALF에서는 주저하지 않고 빨리 시작하고 있습니다.
2. Primary biliary cholangitis (cirrhosis → cholangitis로 이름이 변경되었습니다)
아주 좋은 Figure가 2개 있어 이를 소개합니다. Obeticholic acid가 second line therapy로 소개되고 있습니다.
