[ColonTODAY 121. Diversion colitis]

직장암/대장암이나 게실염 등으로 stomy를 만든 환자에서 distal colon은 변이 통과하지 못하는 상태가 오래 지속됩니다. 대장은 대변이 통과하는 것이 생리적인데, 오랜 기간 변이 통과하지 않으면 또 다른 문제가 발생합니다. 이를 diversion colitis라고 합니다. 무증상부터 다양한 비특이적 증상까지 발생할 수 있으며 내시경 소견도 넓은 spectrum을 보입니다.

직장암 CCRT 후 laparoscopic ISR, loop ileostomy 시행한 한자로 take down 전 sigmoidoscopy 소견.
상기 환자 ileostomy take down 후 diversion colitis가 깨끗하게 좋아졌음.

[Diversion colitis]

Nonspecific inflammatory disorder that occurs in segments of the colon and rectum that are diverted from the fecal stream by surgery.
Characterized by inflammation of the defunctionalized, bypassed colon following surgery.
Most patients are asymptomatic.
Histologic changes of diversion colitis may be seen in the distal colonic segment of 70 to 100 percent of patients with fecal diversion.
The lack of short-chain fatty acids with other luminal nutrients needed by colonocytes or interference with their metabolism by alterations in gut flora may have a role in the development of colitis.
Fewer than 50 percent of patients have symptoms.
Tenesmus, urgency, bloody and/or mucus discharge, abdominal pain.
Observation for asymptomatic patients.
Surgical restoration of intestinal continuity
Short-chain fatty acid enemas, topical 5-ASA agents, topical glucocorticoids.
Proctectomy or sigmoid colectomy with proctectomy for refractory symptoms.
