[일원내시경교실 목요점심집담회 2016-1-14]

1. 위암인데 타장기 질환 (폐암 재발)으로 수술하지 못함

2. 첫 조직검사에서 암으로 확인되지 않았으나 즉시 전원되어 재검을 통하여 진단되었던 보만 4형 진행성 위암 (위암 330)

6개월 전부터 소화불량, 속쓰림, 오심, 구토.

첫 내시경 조직검사에서 암으로 나오지 않아 "mid and lower body에서 edematous, hypermic mucosa, coarse nodularity, 공기주입에도 잘 펴지지 않음 등 소견"이 있어 재검이 필요하다고 의뢰되었습니다. 조직검사를 하였고 전정부 대만에서는 암(signet ring cell carcinoma)으로 나왔으나 암 침윤 부위가 틀림없는 위체상부에서는 암이 확인되지 않았습니다. 보만 4형 진행성 위암에서는 조직검사에서 암으로 확인되지 않을 수 있음을 잘 보여주는 증례였습니다. 미란이나 궤양부위를 잘 찾아서 조직검사를 하는 것이 최선입니다. 수술을 하였습니다. 매우 심했습니다.

7. Resection margin: free from carcinoma, safety margin: proximal 1.5 cm, distal 2 cm
8. Lymph node metastasis : metastasis to 5 out of 50 regional lymph nodes (pN2)
9. Lymphatic invasion : present (++)
10. Venous invasion : not identified
11. Perineural invasion : present
12. Peritoneal cytology : atypical cells
13. AJCC stage by 7th edition: pT4a N2

3. Diffuse large B cell lymphoma
