1. Adenoma with HGD

Stomach, endoscopic submucosal dissection:
Tubular adenoma with high grade dysplasia
1. Location : antrum, anterior wall

3. Size of adenoma : (1) longest diameter, 42 mm (2) vertical diameter, 34 mm
4. Resection margin : negative resection margins(N)

2. 전형적인 위궤양의 치료

'식후 1시간에 우리하게 배가 아프다'는 증상으로 내시경 검사를 받은 환자입니다. 이 증례를 바탕으로 위궤양의 조직검사 및 추적관찰 policy를 살펴보겠습니다.

위각부의 전형적인 A2 stage 궤양이 발견되었습니다. 이 경우는 궤양 edge의 약간 안쪽에서 정확히 조직검사를 2-3개 하시기 바랍니다. 위전정부 대만과 위체중부 대만의 위축되지 않은 점막에서 각각 조직을 1점씩 얻어서 헬리코박터를 위한 Giemsa staining을 의뢰하십시오. CLOtest는 하지 않습니다. 두 검사를 동시에 하면 보험에서 삭감되기 때문입니다 (정말 맘에 들지 않는 정책입니다. 어쩔 수 없습니다).

저는 PPI를 2주 처방하였고 2주 후 외래 방문을 권하였습니다. 강력히 금연을 권했습니다. 2주 후 외래에서 헬리코박터 제균치료 약 1주 (PPI + amoxicillin + clarithromycin)와 추가로 PPI 3주분을 처방하였습니다. 그리고 2주 정도 약을 끊은 상태에서 EGD와 UBT를 할 수 있도록 예약하였습니다.

추적 내시경에서 위각부 궤양이 완전히 호전되어 scar로 바뀌었습니다. 비록 육안적으로 호전되었더라도 암을 배제하기 위하여 scar에서 조직검사 2개를 하시기 바랍니다. 그리고 위전정부 대만과 위체중부 대만에서 Giemsa 염색을 위한 조직을 2점씩 채취하십시오. (UBT가 함께 처방된 환자의 경우는 Giemsa 염색을 위한 조직검사는 skip해도 좋습니다. 두 검사를 하는 것이 더 좋지만 심평원에서 자꾸 삭감하는 통에...... 저는 UBT 혹은 Giemsa 중 하나만 해도 좋은 것으로 하고 있습니다.)

궤양 반흔 조직검사에서는 특이소견이 없었고, UBT와 Giemsa는 헬리코박터 음성으로 나왔습니다. 더 이상 투약은 하지 않았습니다. 다시 한번 금연을 강조하였고 6개월 후 내시경 재검을 권하였습니다. 1년 후에 하셔도 상관없습니다.

3. GIST

Melena로 내원하여 내시경 후 수술 (wedge resection)을 하였습니다. 내시경 조직검사에서 GIST가 확인되었던 경우입니다.

2) mitosis: 88/50 HPF (high powered fields)

7) invasion into mucosa: present
8) resection margin involvement: absent

수술 후 Glivec으로 adjuvant chemotherapy를 하고 있습니다.

4. GIST

Melena로 내원하여 내시경 후 수술 (subtotal gastrectomy)을 하였습니다. 내시경 조직검사에서 GIST가 나오지 않았던 경우입니다.

10) no metastasis in regional lymph nodes

5. Behcet disease

4개월간 지속된 RLQ pain을 주소로 시행한 대장내시경입니다.
