[일원내시경교실 목요점심집담회 2016-3-17]

1. AGC

Cardia cancer는 조기위암 분류나 보만 분류를 붙이기 어렵습니다. 그러나 현재 감독기구에서는 위암의 내시경 진단에서 조기위암 분류나 보만 분류를 붙이지 않으면 검사 자체가 부적절한 것으로 평가하고 있습니다. 부적절한 심평원 평가법에 대한 개선안이 올라간 상태입니다. 당분간은 cardia cancer도 조기위암 분류나 보만 분류를 꼭 붙이도록 합시다. 이 환자의 경우는 "Cardia cancer, AGC Borrmann type III"와 같은 impression을 붙여주세요.

3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : mixed
5. Size : 5.5x5 cm
6. Depth of invasion : invades muscularis propria (pT2)
7. Resection margin: free from carcinoma
8. Lymph node metastasis : metastasis to 2 out of 41 regional lymph nodes
9. Lymphatic invasion : present
10. Venous invasion : not identified
11. Perineural invasion : not identified

2. Ischemic colitis

ESRD로 CAPD 받던 분입니다. 혈변은 없었고 LLQ 통증으로 오셨는데 ischemic colitis였습니다. 그런데 특이한 점은 직장까지 involve된 경우입니다. 전체 ischemic colitis 중 5 % 정도는 rectum도 침범하는 것 같습니다.

3. 위암

조기위암은 조직검사 후 모양이 변경될 수 있습니다. 위암으로 의뢰된 환자의 첫 내시경 사진을 보면 그 변화를 느낄 수 있을 것입니다. 위암의 자연사도 가능하지만 조직검사로 인한 변화 혹은 PPI 투약으로 인한 변화일 수 있습니다. 늘 첫 내시경 사진을 찾아보는 습관을 가집시다.

3. Histologic type : signet-ring cell carcinoma
4. Histologic type by Lauren : diffuse
5. Size : 2x1.3 cm
6. Depth of invasion : invades mucosa (muscularis mucosa) (pT1a)
7. Resection margin: free from carcinoma, safety margin: proximal 2.3 cm, distal 6 cm
8. Lymph node metastasis : no metastasis in 47 regional lymph nodes (pN0)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : not identified

4. TB colitis

48세 남자. 우연히 시행한 대장내시경입니다. A colon에서 아래와 같은 병소가 나왔습니다.

조직검사에서는 focal active inflammation with cryptitis, crypt abscess and erosion으로 나왔으나 AFB smear에서 trace로 나오고 culture positive였습니다.

5. Gastric GIST

Gastrointestinal stromal tumor of low risk of malignant potential:
1) tumor size: 4x3.5 cm
2) mitosis: 1/50 HPF (high powered fields)
3) histological type: spindle (cellular spindle)
4) necrosis: absent
5) cellularity: intermediate
6) cellular atypia: mild
7) invasion into mucosa: absent
8) resection margin involvement: absent.
9) C-kit: Positive with dot-like accenturation
