[EsoTODAY 029 - Upper esophageal adenocarcinoma. 상부식도선암]

검진 내시경입니다. 상부식도(상절치 20-23cm)에서 궤양성 병변이 발견되었습니다. 조직검사는 adenocarcinoma, well differentiated였습니다. CCRT 후 3-field operation을 시행하였습니다. 최종 병리결과는 no residual tumor (status post neoadjuvant chemoradiation therapy)였습니다.

상부식도 선암이라는 점과 선암임에도 불구하고 CCRT 후 no residual tumor였다는 점에서 흥미로운 증례였습니다.

처음 진단되었을 때 내시경 사진을 잘 보시기 바랍니다. 내시경 삽입 과정에서는 암을 발견하지 못하였고, 내시경을 빼는 과정에서 암이 진단되었습니다. 상부식도는 매우 유명한 blind area입니다. 내시경을 천천히 제거하면서 자세히 관찰하지 않으면 상부식도 암은 놓치기 마렵니다. 내시경 천천히 뺍시다.
