[EsoTODAY 030 - Barrett adenocarcinoma]

검진 내시경에서 sentinel polyp 의심으로 조직검사를 하였는데 atypical gland로 나와 추적검사를 통하여 adenocarcinoma로 진단되었습니다. Barrett adenocarcinoma로 판단되어 ESD를 시행하였습니다 (민병훈 교수님).

3. Histologic type : tubular adenocarcinoma, moderately differentiated

5. Size of carcinoma : (1) longest diameter, 9 mm (2) vertical diameter, 4.5 mm
6. Depth of invasion : invades mucosa (muscularis mucosa) (pT1a)
7. Resection margin : free from carcinoma(N), safety margin : distal 8 mm, proximal 9 mm, anterior 12 mm, posterior 14 mm
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent

아무리 sentinel polyp 같아도 처음 발견되었거나 과거와 모양이 다르다면 조직검사를 해야 합니다.

[추가] 화요집담회에서 논의하였습니다. 이풍렬 교수님께서는 단호하게 말씀하셨습니다. "저것은 sentinel polyp이 아니네. 그냥 polyp이지. 조직검사하여 결과를 확인하고 적절한 치료를 해야 하네."
