[SmallTODAY 012 - Duodenal lymphoid hyperplasia]

Functional dyspepsia를 가지고 있는 38세 남자환자의 십이지장 소견으로 구부에 국한된 nodular lesion이 있었으며 조직검사에서는 lymphoid hyperplasia였습니다. 소장의 lymphoid hyperplasia는 내시경상 주로 ileum에서 정상인에서도 발견됩니다. 십이지장이나 공장에서는 아래에 설명한 바와 같이 면역저하질환에서 발견될 수 있으며 별다른 동반질환이 없는 정상인에서도 발견됩니다 (참고: Govoni AF. Benign lymphoid hyperplasia of the duodenal bulb.Gastrointest Radiol 1976;1(3):267-9) 본 환자에서는 몇 가지 기본적인 검사에서 면역억제질환의 증거는 없었습니다. Nodular lymphoid hyperplasia에 대한 Sivac textbook을 옮깁니다.

Patients with acquired hypogammaglobulinemia may develop a nodular mucosal pattern of the small bowel. These nodules are the result of lymphoid hyperplasia, which represents a response to deficiency or absence of IgA. Although the nodules are seen predominantly in the jejunum or ileum, on one occasion they were found endoscopically in the descending duodenum, where they appeared as 1- to 3-mm, soft, polypoid excrescences covering the mucosa. The biopsy specimens showed lymphoid hyperplasia, and aspirates from the duodenum revealed Giardia lamblia, a commonly associated infection
