Patients with acquired hypogammaglobulinemia may develop a nodular mucosal pattern of the small bowel. These nodules are the result of lymphoid hyperplasia, which represents a response to deficiency or absence of IgA. Although the nodules are seen predominantly in the jejunum or ileum, on one occasion they were found endoscopically in the descending duodenum, where they appeared as 1- to 3-mm, soft, polypoid excrescences covering the mucosa. The biopsy specimens showed lymphoid hyperplasia, and aspirates from the duodenum revealed Giardia lamblia, a commonly associated infection
