[16th Recent Update Symposium on Neurogastroenterology & Motility]

제 5회 의편협 (의학학술지편집인협의회) 논문작성워크숍에서 Figure 만들기에 대한 강의 후 연세대 심포지엄에 조금 늦게 참석하였습니다. 아쉽게도 Philip Chiu 선생님의 강의는 듣지 못했고 연세대 윤영훈 선생님과 Nagasaki 대학의 Hitomi Minami 선생님의 강의만 들을 수 있었습니다.

1. Achalasia & POEM (2015년 8월 29일 연세 motility symposium)

1) 윤영훈 선생님 강의 중에서

Submucosal tunnel을 만들 때 점막하층 깊은 부위를 절개해야 합니다. Mucosal injury를 줄일 수 있기 때문이다.
Sigmoid type achalasia에서는 방향을 잡기 어렵습니다. Circulr muscle과 직각 방향으로 진행해야 합니다.
여러 종류의 knife를 시도해 보았지만 selective circular muscle cutting에는 triangle knife가 가장 유리합니다. 최근에는 시작부터 끝까지 triangle knife만 이용하고 있습니다.
Positive pressure ventilation이 필요합니다. Myotomy를 시작하기 전 마취과에 PEEP을 걸어달라고 요청하고 있습니다.

2) Hitomi Minami (Nagasaki University) 선생님 강의 중에서

지난 9년 동안 Showa 대학과 Nagasaki 대학에서 시술한 1,000예의 자료입니다. Minami 선생님은 Inoue 선생님이 세계 최초의 POEM을 할 때 fellow로서 assist를 했습니다. 지금까지 Showa 대학에서 900예 이상 (최근 1년에 200예 이상을 시술하고 있습니다), Nagasaki 대학에서 100예 이상의 POEM이 시술되었습니다.
Myotomy length는 평균 13.6 cm였습니다. (이준행 주. Surgical Heller myotomy에서 myotomy length는 보통 8 cm입니다. 식도로 높게 올라가기 어렵기 때문입니다. 긴 myotomy를 위해서는 수술보다 내시경이 유리합니다.)
Diffuse esophageal spasm에서는 myotomy length를 더 길게 합니다 (20cm 까지).
식도이완불능증에서 식도암 발생률이 높은데 POEM 후 식도암 발생률이 낮아지는지는 아직 불명확합니다.
일본에서 POEM 시술료는 약 15만엔입니다 (우리 돈으로 150만원 정도). 아직 보험급여가 되지 않아서 시술료는 환자 부담인 모양입니다. 입원, 주사, 병실료 등은 보험 급여가 가능하여 환자는 일부만 부담합니다.

일전에 EndoTODAY에서 2015년 Plos One(Minami 2015)에 발표된 식도이완불능증의 새로운 내시경 소견인 pinstripe pattern을 소개한 바 있습니다. Minami 선생님은 그 저널의 저자이셨습니다. 강의 중에서도 이 부분을 소개해 주셨습니다. "Pinstripe Pattern"은 '가늘고 긴 longitudinal longitudinal wrinkcle'입니다. POEM을 하면 myotomy를 시행한 부위의 pinstripe pattern이 없어진다고 합니다. Pinstripe pattern의 kappa 값은 0.61로 food/liquid remnant 소견의 0.69 와 비슷합니다.

강의를 들으면서 다시 한번 느꼈지만, 보험제도(신의료기술 등)가 우리나라 의학 발전을 저해하고 있습니다. 2015년 현재 우리나라에서 POEM은 '보험급여'도 아니고 '인정 비급여'도 아니고 '신의료기술'도 아니고 아무 것도 아닙니다. 현실적으로는 (IRB 통과 후 임상연구 개념으로) 거의 공짜로 시술되고 있습니다. 일본, 중국, 미국, 유럽에서는 식도이완불능증의 표준치료로 인정되는 POEM이 오직 우리나라에서만 불법시술로 취급받고 있습니다. 안타깝습니다.
