내시경집담회. EndoTODAY 이준행

1. Rectal serrated adenoma (3.5cm)

2. AGC

내시경을 배우고 계신 분이 조기위암 같다는 의견을 주셨기에 제가 comment를 하였습니다.

"조기위암이 아닙니다. 전체적으로 mass with central depression 양상이므로 superficial lesion으로 보기는 어렵습니다. Multiple converging folds가 dam formation을 보이고 있으므로 최소한 proper muscle cancer입니다. 게다가 조직검사가 poorly differentiated adenocarcinoma입니다. 조기위암 깊이(depth of invasion) 예측은 differentiated type histology에서 잘 맞습니다 (앞으로는 '심달도'라는 말을 쓰지 않고 그냥 '깊이'라고 부를 것입니다). Undifferentiated type histology (P/D 혹은 SRC) 에서는 depth of invasion이 예상보다 깊은 경우가 많습니다. 저는 이 환자의 내시경 소견을 보고 PM cancer라고 생각하였고, P/D라는 조직검사를 보고 더 깊을 것으로 추정하였습니다. 최종 결과는 multiple lymph node metastasis를 동반한 깊은 암이었습니다."

3. Histologic type : tubular adenocarcinoma, poorly (poorly cohesive) differentiated
4. Histologic type by Lauren : diffuse
5. Size : 3.2x1.6 cm
6. Depth of invasion : penetrates subserosal connective tissue (pT3)
7. Resection margin: free from carcinoma, safety margin: proximal 3.6 cm, distal 9.2 cm
8. Lymph node metastasis : metastasis to 16 out of 36 regional lymph nodes (pN3b), (perinodal extension: absent) (16/36: "5", 0/0; "6", 0/6; "7", 2/4; "9", 2/2; "8a", 6/6; "11p", 0/3; "12a", 0/5; "4sb", 0/0; "1", 0/3; "3,5", 6/6; "4,6", 0/1)
9. Lymphatic invasion : present
10. Venous invasion : not identified
11. Perineural invasion : present
12. Peritoneal cytology : negative
13. AJCC stage by 7th edition: pT3 N3b

3. AGC Borrmann type IV

대장까지 침범된 상태였습니다. 위내시경 조직검사에서 암이 나오지 않아서 EMR까지 했는데도 암이 나오지 않아서 결국 laparoscopy로 조직을 확인할 수 있었습니다. (마지막 내시경: 20개월 전)

보만 4형 진행성 위암에서 erosion이나 ulcer가 없으면 조직진단이 어려울 수 있습니다. 그냥 두꺼워진 주름에서 조직검사를 하면 암이 나오는 경우가 절반 이하입니다. 그러한 경우 bite on bite(우물파기 전법이라고 합니다)를 해도 암이 나오지 않기 쉽고, EMR을 해도 암이 나오지 않을 수도 있습니다. 어쩔 수 없이 수술을 해야만 합니다. 사실 내시경 육안소견이 명확하면 조직 확진을 얻기 위하여 이런 저런 노력을 하기보다는 바로 수술을 하는 편이 훨씬 좋다고 생각합니다.

항상 잘 보고 있습니다. 항상 너무 감사드려요ㅠ.ㅠ/// 더 열심히 공부해서 early cancer들을 잘 잡아내도록 하겠습니다!!! 이번 주 보내주신 Borrmann type IV AGC 케이스를 보고 나니 정말더 열심히 공부해야겠다는 생각이 들면서도 나름 좌절도 했습니다ㅠ.ㅠ Bx.도 잘 나오지 않는 type IV인데, 20개월 전 내시경 소견을 봤을 때 저걸 어떻게 잡아낼 수 있을까 아무리 고민해 봐도 잘 모르겠더라구요. 답은 열공인 것 같습니다. 다시 한 번 항상 감사드립니다.

7. Resection margin: free from carcinoma, safety margin: proximal 1.3 cm, distal 5.5 cm
8. Lymph node metastasis : metastasis to 14 out of 42 regional lymph nodes (pN3a), (perinodal extension: present) (14/42: "2", 1/3; "3", 1/3; "4", 2/6; "5", 4/8; "6", 0/0; "7", 0/2; "9", 1/6; "8a", 1/2; "11p", 1/6; "12a", 3/3; "4sb", 0/2; "1", 0/1)
