[SmallTODAY 007 - Short bowel syndrome after obesity surgery]

2016년 6월 10일 삼성의료원 집담회에서 발표된 삼성서울병원 증례를 소개합니다.

고도비만 수술 후 발생한 short bowel syndrome

비만 수술 후 발생한 부종으로 내원한 30대 외국인. 고도비만으로 두번의 gastric bypass surgery 후 내원 3개월전부터 발생한 both lower leg edema, bruise 로 2차병원 입원 중 r/o protein losing enteropathy 로 전원. 이 환자는 protein losing enteropathy는 아니었고 common channel 길이가 짧아져 발생한 short bowel syndrome이었습니다.

α1-Antitrypsin clearance 검사 결과는 아래와 같이 정상이었습니다.

α1-Antitrypsin, serum : 95.3 mg/dL (ref. 90-200)
α1-Antitrypsin, feces : 8.5 mg/dL (ref. 0-54)
α1-Antitrypsin plasma clearance : 0.18 mL/day (ref. 27 mL/day이하)
공식: Alpha 1-AT plasma clearance = ((stool volume) x (stool alpha 1-AT)) / (serum alpha-1 AT), Alpha-1-antitrypsin (A1A) is resistant to degradation by digestive enzymes and is, therefore, used as an endogenous marker for the presence of blood proteins in the intestinal tract. A1A clearance is reliable for measuring protein loss distal to the pylorus.

Albumin replacement, 영양 공급 (TPN, high protein diet, protein powder, IV iron, vitamin K, vitamin B12 replacement) 등 치료 후 체중도 감소하고 온갖 증상도 호전되어 되의뢰하였습니다.
