심장 판막 수술 후 아스피린과 와파린을 복용하던 중 발생한 melena였고 조직검사에서 signet ring cell carcinoma가 나왔습니다. 병소의 크기를 잘 생각해 보시기 바랍니다.

주름이 끌려오는 뚜렷한 함몰부의 소만쪽으로 넓게 퇴색되고 아주 약간 flat depressed된 부분을 보셨습니까?

Early gastric carcinoma
1. Location : middle third, Center at low body and anterior wall
2. Gross type : EGC type IIb+IIc
3. Histologic type : signet-ring cell carcinoma
4. Histologic type by Lauren : mixed
5. Size : 3.5x2.9 cm
6. Depth of invasion : invades submucosa (sm3) (pT1b)
7. Resection margin: free from carcinoma, safety margin: proximal 1.9 cm, distal 8.7 cm
8. Lymph node metastasis : no metastasis in 45 regional lymph nodes
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : not identified
12. AJCC stage by 7th edition: pT1b N0

3. Rectosigmoid cancer (signet ring cell carcinoma) with multiple metastasis

두 달 전부터 배변이 잦아지면서 잔변감, 옆구리 통증 및 체중감소가 발생하였고 left cervical lymph node가 만져짐.

첫 내시경 조직검사에서 focal active inflammation with lymphoid aggregate가 나왔으나, 재검에서 signet ring cell carcinoma within lymphatic spaces; either primary or metastatic 소견이었습니다. CEA가 383로 상승되어 있었습니다. PET에서는 뚜렷하지 않았으나 chest CT와 abdominal CT에서는 multiple metastasis가 있었습니다. 위내시경은 정상.
