1. Early gastric cacner

ESD를 하였지만 아쉽게 수술이 필요한 것으로 나왔습니다.

Stomach, endoscopic submucosal dissection:
Early gastric carcinoma
1. Location : body, greater curvature
2. Gross type : EGC type IIb+IIc
3. Histologic type : signet ring cell carcinoma >> tubular adenocarcinoma, moderately differentiated (20%)
4. Histologic type by Lauren : diffuse
5. Size of carcinoma : (1) longest diameter, 31 mm (2) vertical diameter, 26 mm
6. Depth of invasion : invades submucosa, (depth of sm invasion :600㎛) (pT1b)
7. Resection margin : free from carcinoma(N), safety margin : distal 9 mm, proximal 3 mm, anterior 8 mm, posterior 12 mm, deep 600㎛
8. Lymphatic invasion : present
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: present

2. Superficial esophageal cancer

조직검사에서는 high grade dysplasia로 나왔으나 최종 병리결과는 SM invasion을 보인 37mm 크기의 식도암이었습니다.

3. Mantle cell lymphoma

8년 전 colon mantle cell lymphoma로 R-HyperCVAD 항암치료 후 NED state였던 분의 최근 대장내시경입니다. 과거에도 그랬고 이번에도 마찬가지로 작은 용종으로 보이는 곳에서 조직검사를 했는데 모두 mantle cell lymphoma로 나왔습니다. Mantle cell lymphoma의 전형적인 소견인 lymphomatous polyposis라고 할 수 있습니다.

4. Pseudomembranous colitis

80대 환자로 요양병원 입원 중 UTI로 7일간 항생제 사용 후 혈변. 조직검사 : focal active inflammation and necrotic detritus, C. difficile toxin (+)
