절대 적응증에 해당되더라도 막상 ESD를 해보면 생각보다 깊거나 림프선에 침윤된 경우를 만납니다. 적응증을 잘 잡아도 ESD 후 10-15% 정도는 수술이 필요합니다.

위식도접합부암에 대한 ESD 후 수술이 필요하다고 말씀드리면 환자들은 대단히 실망합니다. Total gastrectomy를 해야 하기 때문입니다. 그러나 어쩔 수 없지요. 예측의 정확도가 충분히 만족스럽지 못하기 때문이니까요...
