위체중부 signet ring cell carcinoma인데 소만쪽으로 병소의 범위를 알기 어렵다는 질문을 받았습니다. 애매한 곳을 조직검사하여 암이 아닌 것으로 나오기는 했지만 확신이 서지 않는다는 것이었는데요... 아래 그림과 같이 답했습니다.

이런 경우 부분 위 절제술을 하게 되면 remnant stomach이 매우 작을 수 밖에 없는데... 그래서 저는 total gastrectomy를 권하는데... 외과에서는 total 보다는 near total 을 선택하는 경우가 많습니다. 위를 조금 남기는 것이지요. 우리나라 사람들이 워낙 total gastectomy를 싫어하는 정서가 있어서 외과의사들의 수술 방법 선택에 영향을 미치는 것 아닌가 추정해 봅니다.

수술 후 최종 병리결과입니다.

3. Histologic type : tubular adenocarcinoma, poorly (solid) differentiated with signet ring cell component (20%)

6. Depth of invasion : invades mucosa (muscularis mucosae) (pT1a)
7. Resection margin: free from carcinoma, safety margin: proximal 3 cm, distal 7.8 cm
