위암과 간암이 있어 모두 수술하였습니다.

AGC mimicking EGC라는 병리 결과를 받았습니다만 내시경 소견을 검토해 보았을 때 충분히 AGC일 수 있는 모양인 경우입니다. Borrmann type I, II, III, IV라고 명명할 수 없는 AGC는 매우 다양한 특성을 가지고 있습니다. 그 중에서 흔한 것은 (1) small EGC인데 병소의 depth of invasion이 깊은 경우이며 (2) 이처럼 AGC Borrmann type IV라고 할 수 없지만 병소가 넓고 바닥면 함몰부위가 불규칙한 경우도 있습니다. 일반적으로 넓다고 AGC인고 얕다고 EGC인 것은 아닙니다. 특히 융기형 병소에서는 넓이와 depth of invasion간에는 별다른 관계가 없기도 합니다. 그러나 함몰형 병소에서는 함몰부의 깊이와 함몰부의 넓이가 depth of invasion과 어느 정도는 관련되어 있을 것이라는 느낌을 가지고 있습니다.

물론 제 느낌이니까 근거자료를 대라고 한다면 할 말은 없습니다. 우리들이 하는 일 중에서 자료화하기는 어렵지만 (=논문으로 만들기는 어렵지만) practical하게는 크게 도움이 되는 것들이 많다고 생각합니다 (expert opinion이라고 부르는 것들입니다. Evidence-based medicine 분야에서는 거의 무시되는 것이지만 저는 expert opinion의 신봉자입니다). 특히 science인지 art인지 명확하지 않은 내시경 분야는 더욱 그러합니다. 도제교육이 중요할 수 밖에 없습니다.

6. Depth of invasion : extension to proper muscle (pT2a)
7. Resection margin: free from carcinoma, safety margin: proximal, 4 cm; distal, 3.2 cm
8. Lymph node metastasis : metastasis to 12 out of 41 regional lymph nodes (pN2)
