20대 여성의 위체하부에서 발견된 궤양성 병소입니다. 조직검사에서는 signet ring cell carcinoma로 나왔으며 수술을 시행하였습니다. Pathologic staging을 추측해 보시기 바랍니다. 아래 답을 보시지 마시고 우선 나름대로의 생각을 메모장에 써 보십시오.

1) Histologic type : tubular adenocarcinoma, poorly differentiated
2) Histologic type by Lauren : intestinal
3) Size : 4.2x3.8x0.9 cm
4) Depth of invasion : extension to subserosa (pT2b)
5) Resection margin: free -- safety margin: proximal, 2.3 cm; distal, 7.8 cm
6) Lymph node metastasis: metastasis to 11 out of 56 regional lymph nodes (pN2)
7) Lymphatic invasion: present
8) Venous invasion: present (intramural)
9) Perineural invasion: present
놀랍지 않습니까. 젊은 환자에서의 위암은 노령의 위암과 분명히 다르다고 생각합니다. 작아 보이는 위암에 대해서도 적극적인 치료법을 선택해야 한다고 생각합니다.
