일전에 fundus에 blind area가 있다는 점을 말씀드린 기억이 납니다. 이번에 바로 그 부위에 위암이 발생한 환자를 소개합니다. 좌측 사진과 같은 retroflection view에서 좌측 하단의 irregular한 병소는 miss하기 쉽다고 생각됩니다. 우측 cartoon (1)에서와 같은 이유로 fundus의 대만에 있는 병소는 retroflection에서 잘 보이지 않기 때문입니다. 이런 위치의 병소를 놓치지 않기 위해서는 cartoon (2)에서와 같이 내시경을 빼면서 위체상부 대만과 fundus의 대만부위를 잘 관찰해야 하겠습니다.
Advanced gastric carcinoma ;
1. Location : upper third, Center at fundus and posterior wall
2. Gross type : Borrmann type 3
3. Histologic type : tubular adenocarcinoma, poorly differentiated
4. Histologic type by Lauren : diffuse
5. Size : 4.5x4x0.8 cm
6. Depth of invasion : penetrates serosa (pT3)
7. Resection margin: free from carcinoma, safety margin: proximal, 3 cm; distal, 16 cm
8. Lymph node metastasis : metastasis to 30 out of 63 regional lymph nodes (pN3)
9. Lymphatic invasion : present
10.Venous invasion : present (extramural)
11.Perineural invasion : present
