근위전정부의 작은 위암이었습니다. IIa+IIc형이었고 조직검사에서 signet ring cell carcinoma with mucin production으로 보고되어 수술을 시행하였습니다. 그런데 깊고 큰 진행성 위암으로 나왔습니다.
조직검사입니다. Signet ring cell carcinoma nest가 군데군데 보입니다. 수술 병리는 아래와 같았습니다. 놀랍게도...
Advanced gastric carcinoma
1. Location : lower third, Center at antrum and greater curvature
2. Gross type : Borrmann type unclassifiable
3. Histologic type : signet-ring cell carcinoma
4. Histologic type by Lauren : diffuse
5. Size : 4.2x3.7
6. Depth of invasion : penetrates subserosal connective tissue (pT3)
7. Resection margin: free from carcinoma, safety margin: proximal 4.9, distal 2.5 cm
8. Lymph node metastasis : no metastasis in 33 regional lymph nodes (pN0)
9. Lymphatic invasion : present
10. Venous invasion : not identified
11. Perineural invasion : present
12. Peritoneal cytology : negative
13. AJCC stage by 7th edition: pT3 N0
Sigent ring cell이 뭉쳐있는 것은 이 부분 뿐이었습니다. 점막층에 국한된 작은 위암처럼 보입니다.
Signet ring cell이 nest를 이루면서 subserosal connective tissue까지 넓게 침윤되어 있었습니다. Mapping하여 전체 크기를 측정하였을 때에는 4.2 x 3.7 cm이었습니다.
Signet ring cell carcinoma는 무섭습니다. 함부러 ESD할 일은 아니라고 생각합니다. 조심하고 또 조심하고...
