오래된 자료를 뒤적이다가 만난 환자입니다. 2010년 증례이니 벌써 세상이 반 이상 바뀌었겠네요. 50대 여성의 fundus cancer였는데 매우 작았고 내시경으로 접근이 어려워 total gastrectomy를 했는데 역시나 미소위암으로 나왔습니다.
Total gastrectom 이외의 다른 option은 없었을까 고민해봅니다. 최근 시도되고 있는 proximal gastrectomy with double tract reconstruction, endoscopic full thickness resection (+ laparoscopic repair), wedge resection and sentinel node dissection 등 아직 표준은 아니지만 고려해 볼 수 있는 술기가 몇 가지 떠오릅니다.

1. Location : upper third, Center at fundus and greater curvature
2. Gross type : EGC type IIb
3. Histologic type : signet-ring cell carcinoma
4. Histologic type by Lauren : diffuse
5. Size : 0.4x0.2 cm (see note)
6. Depth of invasion : invades mucosa (lamina propria) (pT1a)
7. Resection margin: free from carcinoma
8. Lymph node metastasis : no metastasis in 37 regional lymph nodes (pN0)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : not identified
12. AJCC stage by 7th edition: pT1a N0
